Description

This position is responsible for providing physical therapy services to patients pursuant to the established treatment plan.

Ensures the efficient delivery of physical therapy services, hereinafter referred to as 'services' while at a minimum adhering to state and federal laws, state and federal rules, accrediting body standards, accepted physical therapy practices, ethical standards, agency policies and procedures, the agency's mission and values, the agency's code of conduct, and business decorum.

  • Implements patient/client physical therapy regiment pursuant to the established plan of care and conducts visits.
  • Accurately completes weekly schedule and conducts visits ensuring compliance with established visit frequencies.
  • Ensures the start and completion of visits are independently documented using telephony or other prescribed method.
  • Identifies self as a representative of the agency to patients/clients and their families at the start of visits, as necessary.
  • Administers prescribed physical therapy treatment programs for patients/clients to restore function, relieve pain, and prevent disability following disease, injury or loss of body part.
  • Administers manual therapeutic exercises to improve or maintain muscle function, applying precise amounts of manual force and guiding patient's body parts through selective patterns and degrees of movement.
  • Instructs, motivates and assists patients/clients in non-manual exercises, such as active regimens, isometric and progressive resistive, and in functional activities using available equipment and assistive and supportive devices, such as crutches, walkers, canes, orthoses and prostheses.
  • Administers treatment involving application of physical agents, such as heat, light, cold, water and electricity.
  • Administers traction and massage.
  • Evaluates, fits and adjusts prosthetic and orthotic devices and recommend modifications to the orthotist/prosthetist.
  • Observes, documents, and reports to the Physical Therapist the patient's/client's treatment, response and progress.
  • Instructs patients/clients and family on specific issues within the physical therapy program.
  • Works to maximize patient/client outcomes.
  • Ensures the accurate completion of all visit documents in a timely manner.
  • Ensures the accurate documentation of visits in a timely manner.

Attends and participates in meetings and activities to effect quality and performance improvement.

  • Ensures competencies are completed in a timely manner.
  • Participates in agency sponsored continuing education opportunities.
  • With supervisor, identifies continuing education opportunities to improve job-related knowledge and skills.
  • Takes an active role in learning changes to state and federal laws, state and federal rules, accrediting body standards, accepted physical therapy practices, ethical standards, agency policies and procedures, the agency's mission and values, the agency's code of conduct, and business decorum.
  • Takes an active role in learning existing, new, and updates to technology utilized by the agency, including but not limited to patient/client care software, telehealth, and the Microsoft office suite.
  • Attends and participates in various meetings, including but not limited to staff meetings.

Effectively represents the agency in a professional manner.

Communicates significant events and other data necessary for timely and accurate decision-making to the appropriate individuals.

 
Position Requirements

Educational Requirements / Work Experience

  • Graduate of an accredited physical therapy assistant curriculum REQUIRED.
  • License to practice as a physical therapy assistant in the State of Illinois REQUIRED.
  • Certified to perform Cardio Pulmonary Resuscitation PREFERRED.  Certified to perfor CPR within six (6) months of hire REQUIRED.
  • At least one (1) year of experience practicing physical therapy PREFERRED.
  • Previous experience practicing physical therapy in a home health setting PREFERRED.

Physical Requirements

  • Ability to lift 20 lbs above one’s head.
  • Ability to perform a maximum assist for stand pivot transfer.
  • Ability to bend, sit, walk and write.
  • Manual dexterity.
  • Visual/hearing ability sufficient to comprehend written/verbal communication.
  • Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
  • Ability to deal effectively with stress.
