CLASS SUMMARY:

This class provides assessment; outreach; prevention services; coordination; referral and follow up; and quality assurance for residential substance use disorder treatment services for pregnant and postpartum women and their children. Ensures the program meets the census targets according to the current budget. Promotes the meaningful participation of family members, community supports, and stakeholder agencies in crafting innovative interventions, identifying and mobilizing resources, supervising clients and monitoring outcomes. Ensures assigned clients are served in the least restrictive and least intensive level of care that meets their identified needs. Activities include outreach, advocacy, counseling and crisis intervention, daily living support, and similar functions.

DISTINGUISHING ELEMENTS OF THE CLASS:

I. IMPACT: The incumbent's work has impacts on the quality, responsiveness, and efficiency of substance use disorder treatment for pregnant and postpartum women and their families. The incumbent works directly with referral sources, medical care providers, treatment facilities, correctional facilities, attorneys and other Community Services Boards to facilitate program admissions. The incumbent's work directly affects the ability of the agency to obtain Medicaid reimbursement for delivered services. The incumbent work performance directly affects the program financial objectives.

II. LATITUDE: Day-to-day work is performed independently with regular supervision and follow-up from the Program Manager. Work is guided by an outreach/marketing plan, the Substance Abuse and Prevention Treatment block grant's women's set-aside, agency policies, diagnostic manuals, and the standards of the profession.

III. RESPONSIBILITY FOR THE WORK OF OTHERS: No responsibility for the work of others.

IV. INTERPERSONAL CONTACTS: Work involves direct and continuous contact with clients and their families. The incumbent interfaces with a variety of others, internal and external, to the agency. These include: physicians, social service personnel, therapists, social workers, psychiatrists, court and law enforcement officials, other agency staff, case managers from other agencies and others. The incumbent will conduct and report prevention activities in the state data base.

V. DIFFICULTY: Appropriately relating to a variety of professionals, agencies and clients and families requires use of judgment and discretion. Emergency situations involve timely and responsive decision making. Work involves the application and interpretation of diagnostic norms and therapeutic standards to clients with a wide range of emotional disturbance. Assessment involves acute observation and analysis of current and historical behavioral presentations, medical and social histories and mental status data. Work requires the balancing of administrative norms/expectations, client needs and ethics and standards of the agency and profession.

VI. COMMUNICATIONS: Incumbent will communicate effectively, in writing and orally, to other professionals, clients and family members, and staff of other service agencies. Incumbent must also prepare complex documentation, technical records, and reports, treatment plans.

VII. EQUIPMENT/FACILITIES RESPONSIBILITY: Incumbent operates personal vehicle to visit client and contact community resources. Must be well-versed in the use of Windows-based business software applications.

VIII. WORKING CONDITIONS: Some work is done in an office environment. Work frequently involves traveling and visitation to treatment sites, correctional facilities, and other agency offices. Incumbent may be exposed to hostile and/or aggressive client behavior, presenting a personal risk of bodily harm. Requires 24 hours per day, 7 days per week on-call coverage.

IX. KNOWLEDGE, SKILLS AND ABILITIES (KSA'S):

Skilled at developing and implementing an outreach/marketing plan to meet the census goals.

Excellent interpersonal and facilitation skills.

Ability to form accurate diagnostic impressions on the DSM IV and determination of patient placement criteria.

Good knowledge of community services and resources.

Ability to establish and maintain good working relationships with clients, families, staff, agencies, and community supports.

Ability to coordinate medical care for pregnant and postpartum women and children.

Ability to maintain and monitor accurate and up-to-date clinical records and reports.

Excellent oral and written communication skills.

Good knowledge of crisis intervention principles and practices.

Thorough knowledge of substance use disorder diagnostic assessment, therapeutic counseling theories, and principles.

Thorough knowledge of Medicaid regulations.

Ability to maintain client confidentiality in accordance with HIPAA, 42 CFR, Part 2 and other regulations.

Ability to work as an active member of a treatment team.

X. LICENSURE: Certified Substance Abuse Counselor and Valid Virginia driver's license.

XI. PRINCIPLE DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Performs case management, outreach, and prevention services.

Obtain all documentation needed for program admission.

Assess client functioning level, assign diagnosis, and determine eligibility in accordance with the American Society of Addiction Medicine Patient Placement Criteria.

Mobilize and facilitate service team and treatment team meetings, empowering and encouraging member participation.

Develops a marketing/outreach plan which ensures the programmatic objectives and outcomes are continuously maintained.

Actively involve client, and their families in the process of treatment planning, transition and reintegration into the community.

Coordinate with current and designated placements to insure optimum transition into post discharge care.

Identify and refer to appropriate community-based services to meet clients and family needs.

Document rationale for admission, continuation, and discharge of current level of care, citing behaviorally specific interventions, successes and other treatment needs.

Prepare correspondence, program and treatment reports as required.

Provide clinical consultation.

Facilitate and attend client appointments as needed.

Actively participate in group and/or individual supervision.

Conduct parenting education and other groups.

Develop a community network team to address the needs of families impacted by parental substance use.

Serves as the liaison to CSBs, correctional facilities, courts, probation & parole.

Performs related duties as assigned.

Transports clients as necessary.

XII. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. Some walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, and handling, sitting, standing, pushing, and pulling.
