SUMMARY: A Case Manager at Open Door Community Health Centers provides integrated, supportive and enabling services to health center patients. Working with the health care team, the Case Manager acts as patient advocate within the context of health center operations and the health goals set by primary medical and behavioral health care providers to address a variety of non-clinical social determinants of health, including housing, finances, food security, social support and health maintenance. The Case Manager is are responsible for providing health education services to patients according to established program guidelines. The Case Manager assists patients access community resources and social services to support their treatment needs and improve overall health status. The Case Manager maintains knowledge of and relationships with community agencies and resources and facilitate patient referrals as necessary to drug and alcohol programs, mental health providers, schools, child care, employment and training programs, food banks, housing assistance, disability assistance and other community sources of support based on patient need. Open Door is looking for a committed and knowledgeable individual who is willing and able to work with diverse patients on a case by case basis. Bachelor's degree in social work, sociology, psychology or human services required; MSW or related advanced degree preferred.
