Catholic Charities of Trenton is a leading provider of Behavioral Health services in Mercer and Burlington Counties. We are currently seeking a part-time (19 hour a week) Licensed Clinical Social Worker (LCSW) in our NEW Integrated Health Center on the campus of Capital Health Primary Care Medical Practice in Trenton. We provide high quality behavioral health counseling to patients within the Capital Health Primary Care Medical practice. JOB SUMMARY: This position is responsible for conducting diagnostic assessments and providing individual, group and family therapy to meet the needs/goals of medical practice patients. ESSENTIAL FUNCTIONS Conduct initial and ongoing assessments involving the collection, analysis, and recording of significant bio-psychosocial information. Makes diagnoses Provide individual, family and group counseling to consumers as needed. Provides information sessions for individuals and groups to reduce resistance to mental health treatment and enhance engagement Collects specimens for drug use analysis Maintains regular therapeutic contact with clients Treatment plans for clients on medication prescribed by Primary Care Medical Program staff will be done in consultation with the prescriber. Enters reports and assessments into information technology system as indicated by the Program Director Maintains accurate and up-to-date case records in accordance with agency standards, accreditation and funding source requirements Coordinates treatment with relevant staff both inside and outside the agency and within the group medical practice. Advocates for consumers and makes indicated referrals Clinicians are expected to spend 65% of their time in face to face consumer contact and to provide routine productivity reports to their supervisor. All schedules, statistical reports/logs, and clinical reports are to be submitted within proscribed timelines. Documentation should be completed in Anasazi (electronic health record) within 24 hours of service delivery. Participate in the Quality Improvement process and specific quality initiatives for the program. Utilize supervision and training as opportunities for enhancing professional competence. Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, and professional standards. Other related duties as assigned. KNOWLEDGE/SKILLS/ABILITIES: Computer literate. Available to work 1 evening a week, per medical practice needs. |
