Community Integration Direct Support Professional

EXAMPLES OF JOB DUTIES:

Individual Related Duties:

  1. Supervise and assist individuals in completion of daily (M-Th) written schedule of activities and routine such as personal care, meal assistance, transfer, lifting, etc., ensuring full participation of individuals, as per individual need and according to their SP and ISSP, completing documentation as per agency guidelines.
  2. Implement and document Individual Service and Support plans, safety plans, and behavior management plans as stated, documenting after completion using progress notes and data collection forms specified per individual.
  3. Maintain required paperwork, including individual ID notes, CP logs, CIE documentation, vehicle attendance, mileage logs, incident reports, etc. as per agency guidelines.
  4. Be aware of individual behavioral issues, providing appropriate intervention, counseling, and follow-up for unusual behaviors or reactions as needed. Document unusual behaviors/reactions in case file and using behavior tracking and/or incident report form as per agency guidelines.
  5. Promote community integration in use of public transportation, use of good pedestrian skills, knowledge of neighborhood, and use of public transportation whenever possible.
  6. Participate as a member of Interdisciplinary Team, attending individual meetings and completing skills assessments as assigned, giving input regarding observations/recommendations of individuals progress and needs.
  7. Assure that individuals get needed medical attention by familiarizing self with individual medical concerns, documenting medication administration and any medical concerns in individual notes as per agency guidelines, responding to emergency situations as per emergency face sheet in individual file.
  8. Evaluate individual skills using community participation or vocational evaluative tools prior to staffing or upon request.
  9. Cooperate with and support residential staff including transportation or related training to and from program as requested, maintaining good public relations and communication with other programs, and notifying CCA office of questions or concerns.

Facility Related Duties:

  1. Ensure the day program area is maintained in a clean and healthy manner, meeting guidelines on facility checklist and HUD on ongoing basis and assisting with plan of correction in deficient areas.
  2. Conduct and document monthly safety drills using specified procedures and safety drill log, assisting residence in increasing independence in evacuation skills and general safety knowledge.
  3. Notify supervisor of maintenance or supply needs obtaining such needed supplies that are stocked at CCA or submitting a list of non- stocked supplies to PM.
  4. Maintain agency vehicles to include cleanliness, fuel, and monthly safety inspections, submitting required inspection form to PM.
  5. Submit a request for funds to the benefits coordinator, one day prior to activities, obtaining and returning store receipts for all purchases.

Agency Related Duties:

  1. Record actual hours worked daily on specified timesheet as per agency guidelines, having available for supervisor’s approval on last day of pay period or as otherwise directed. All leave requests must be submitted in writing and approved by supervisor prior to leave, as per agency guidelines.
  2. Record all mileage using specified forms as per agency guidelines and submitting with time sheet on last day of pay period for approval.
  3. Attend staff meetings and scheduled inservice as requested by supervisor and meeting agency inservice requirements, completing all assigned tasks within designated time frames.

Expectations for Completion of Job Duties:

  1. Display professional appearance and behavior, interacting with other staff, individuals, parents, and service providers in a cooperative and friendly manner as observed by supervisor.
  2. Punctual and dependable at reporting to work. (Absenteeism and lateness does not result in adverse affects on the effective operation of the facility).
  3. Perform job duties with a positive attitude, responding to peak work period without loss of quality, accuracy and thoroughness.
  4. Uphold individual rights by following individual rights and confidentiality guidelines and agency abuse, neglect, mistreatment, and exploitation policy daily.
  5. Provide positive role model to staff and individuals always, avoiding use of profanity or derogatory slang, arriving to work in proper attire, modeling good problem solving and social interactions.
 
Position Requirements

High School Diploma or GED. Previous experience in related field preferred. Must have valid driver’s license, reliable transportation, and proof of insurance. Motor vehicle report meeting driving requirements required prior to employment and are to be updated upon request.

Supervision Received: Works directly under supervision of the Community Services Manager; Program Director or CEO is available to resolve unusual work issues.
