POSITION SUMMARY

Integrated Employment (IE) Program Facilitators provide community-based support, supervision and instruction to participants with intellectual & physical disabilities. IE Program Facilitators also provide administrative, operational and personnel support to the IE Program Manager (PM).

ESSENTIAL FUNCTIONS

Administrative Support

  • Develop and implement current, functional and age-appropriate curriculum for participant activities.
  • Develop, implement, modify and review data tracking sheets for each participant.
  • Maintain information in the Program Data Management System.
  • Create, maintain and update field packs, job binders, new participant packets, participant files/records and case files.
  • Report wages for participants to SSA.
  • Complete Special Incident Reports (SIR) and submit to the appropriate agencies/persons in the PM's absence.
  • Distribute bus passes to staff and participants and maintain bus pass log.
  • Track, calculate and compile monthly, semi-annual and annual data.
  • File, purge and archive program information and SSA wage reporting forms, per program guidelines.
  • Complete and submit billing support information.
  • Take notes at Program Meetings and distribute/file, as needed.
  • Document medications and side effects in collateral and community binders.
  • Assist with the completion of programmatic reports, documentation, forms and paperwork.

Operational & Programmatic Support

  • Provide support, supervision, motivation and instruction to participants in the community and at job sites.
  • Assist in the development & distribution of participant schedules and calendars.
  • Ensure that the participants' work and training schedules are followed and documented.
  • Assist with new employee orientation at new job sites.
  • Meet independent employment site dress code and work quality requirement standards.
  • Register individuals at various community colleges.
  • Perform and document spot checks.
  • Complete semi-annual ISP process including mailing and follow-up.
  • Oversee completion of PFPs for each participant.
  • Establish data tracking and other supports for participants at new job sites.
  • Collaborate with Interdisciplinary Team (IDT) individuals.
  • Function as acting Program Manager, when necessary.
  • Serve as a Safety Representative on the Safety Committee.
  • Assist participants with self-help needs, including support in the areas of eating, restrooming and grooming.
  • Diffuse crisis situations and provide crisis intervention, as necessary.
  • Conduct safety drills with staff and participants.
  • Assist with supervision and training of staff; report potential staff issues to Program Manager (PM).
  • Follow-up with staff deadlines for paperwork responsibilities.
  • Effectively relay issues and concerns to the Program Manager.
  • Attend all required meetings and in-services.
  • Complete all required training.
  • Support fundraising activities of the organization.
  • Comply with all Employment & Community Options policies and procedures.
  • Conserve company resources.
  • Other duties, as assigned.
 
Position Requirements

Education

  • High School Diploma, equivalent or GED.
  • Bachelor’s Degree in Human Services or related area (preferred).

Experience

  • Two years hands-on experience with individuals with a wide range of intellectual & physical disabilities, including individuals with the most severe challenges.
  • Experience in community-based instruction in critical life needs areas (preferred).
  • Experience in assisting individuals in the use of adaptive devices and communication aids (preferred).

Required Skills

  • Ability to communicate effectively both orally and in writing.
  • Ability to conduct training.
  • Professional English writing ability, including usage, grammar, spelling, punctuation and vocabulary.
  • Proficient use of basic office equipment, Microsoft Word and Excel.

Other Requirements

  • Immediate access to a vehicle with working seat belts, valid driver’s license, current auto insurance and acceptable DMV record.
  • Ability to travel within and throughout the assigned program area in which services are provided.
  • Ability to travel within and throughout the county on a quarterly basis to attend meetings, trainings, etc.
  • Successful completion of drug screen, background check and physical evaluation.
  • CPR/First Aid Certification (preferred).

PHYSICAL DEMANDS & WORK ENVIRONMENT

This position requires the employee to regularly stand, walk, sit, talk and hear. They must be capable of writing, using a computer keyboard, telephone and related office equipment. Ability to lift 25lbs. and transfer 75lbs. required. 

The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

The employee will be working in community environments, employment sites and office environments. The employee will experience normal office conditions with a well-lighted work area and minor noise from standard office equipment. Exposure to blood borne pathogens could occur during restrooming, feeding, or when administering first aid to participants. Hepatitis B vaccines are offered at no cost to each employee.

Wage $17.00 - $20.00 per hour + benefits Wage is determined by education and experience in the field.
