GENERAL DESCRIPTION OF POSITION
Prepares and provides consultation, training, and instructional assistance to Pathfinder staff, individuals served, parents, and/or legally responsible person of the consumers we serve. Flexible schedule.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Verify change out list is correct and up to date, notifies pharmacy of changes.
2. Administers prescription injections and treatment medications when necessary, if qualified.
3. Verifies doctor's orders are filled correctly by pharmaceutical company before delivery to site.
4. Assists in the design and integration of individual objectives as part of the overall service delivery process.
5. Participates in Multiple Agency Plan Meetings as needed within the Residential and the ACS Waiver departments.
6. Provides information and assistance to persons responsible for developing the consumers' service plans.
7. Keep up with immunizations, blood level appointments, and preventative test for individuals served.
8. Trains direct care staff and family members on implementing integrated community service instructional strategies as related to health.
9. Coordinates and/or completes and submits required quarterly documentation to DDS for Medicaid Waiver (Nurse) Consultation as directed.
10. Will provide assessment and program plans for waiver crisis intervention services if qualified.
11. Prepares proper documentation for services that are billable under ACS Waiver Consultation.
12. In accordance with the Nurse Practice Act, coordinates, tracks, trains, oversees and/or documents appropriate paperwork regarding The Consumer Direct Care Bill as directed.
13. Prepare assessments, evaluations, reports, and physical on individuals served and facilities as needed.
14. Will report and assist with corrective action in the event of medication errors, survey recommendations, etc.
15. Will monitor and/or track client medication passes with new staff to ensure client competence in self administration of medication and training adherence.
16. Oversees or tracks Personal Care Program by providing training to Personal Care Aides for certification, provides additional continued education hours required for re-certification, completes Personal Care Assessment and Service Plan, and completes Personal Care record reviews and program observations.
17. Develops and provides training to individuals served on health, hygiene, and sanitary living conditions.
18. Will assist in selection of additional nursing services as needed or desired.
19. Provides and maintains documentation of current professional license to be on file with Pathfinder, Inc.
20. Will work in a cooperative team spirit with all Pathfinder, Inc departments and employees, parents, legal representatives, and consultant staff.
21. Attends meetings and training sessions as directed.
22. Perform any other related duties as required or assigned.
