Share Email Opening Title Case Manager - Children’s Mental Health Division  Description The Children's Mental Health Division is currently seeking a Mental Health Case Manager. The Case Manager will be responsible for the provision of case management services to consumers. These services are provided both in the office and extensively in the community. Services include the assessment of consumer needs via a review of medical, social, educational, psychological evaluation and the creation, implementation, monitoring and revision of individualized service plans. Collaborates and links consumers with other service providers as needed and monitors these services. Advocates for consumer preferences and service needs. Interfaces with a wide variety of professionals, programs and agencies to deliver services. Provides crisis intervention services, as needed. Attends staff meetings, clinical staffing's, and team meetings. Maintains electronic medical records in accordance with pertinent standards and completes documentation within timeframes established by Medicaid. Maximizes re-imbursement potential.   Position Requirements The preferred candidate will possess a Masters degree in Social Work, Psychology or Counseling with one or more years of experience and eligible for registration of supervision with the applicable licensing board within 3 months of hire.  Consideration will be given to individuals with a Bachelor’s degree in Social Work, Human Services or Psychology with at least one year of experience.   Full-Time/Part-Time Full-Time   Open Date 9/14/2017   Close Date 9/29/2017   About the Organization Join the Fearless!
