Description

PURPOSE:
Provide training and support to DDA (Developmental Disabilities Administration) and KMHS staff managing shared clients who are experiencing challenges and/or in crisis.
Facilitate intake and situation-specific evaluation and referral services for clients in a community mental health setting.

RESPONSIBILITIES AND DUTIES:

  • Attend all DDA regional liaison meetings. Attend treatment team meetings at KMHS as necessary. Coordinate case staffing when required.
  • Work closely with KMHS Resource Management and community professionals on resource options.
  • Facilitate communication between KMHS care coordinators and DDA staff to ensure treatment continuity and successful integration in the community.
  • Demonstrate knowledge in the use of the current Diagnostic and Statistical Manual of Mental Disorders. Possess the ability and knowledge necessary to work effectively with complex cases involving multiple diagnoses, substance abuse issues, developmental disabilities and medical conditions.
  • Demonstrate strong verbal and written skills. Submit required paperwork in a timely way.
  • Work collaboratively with community partners. Work closely, as needed, with family members and advocacy support groups.
  • Provide liaison services between DDA and other community resources.
  • Establish and maintain effective and positive relationships with KMHS and DDA staff, collaborating with staff frequently on the best treatment plan strategies. Appraise resource management work group of barriers with challenging. Co-ordinate with KMHS treatment coordinators in relation to shared DDA clients.
  • Facilitate communication between KMHS treatment coordinators and DDA staff to ensure treatment continuity and successful integration in the community.
  • Attend resource management meetings at KMHS on a routine basis.
  • Collaborate with the Access team and Resource Management to provide timely intake into services. Arrange screenings and intakes for DDA clients when needed.
  • Request, gather, and coordinate information from all sources on prospective clients in order to facilitate appropriate treatment decisions.
  • Provide crisis intervention training as indicated by client need.
  • Provide information and referral to clients not accepted into services. Recommend clients to other agency services as indicated and provide direction to service providers regarding aspects of care to be managed.
  • Document all clinical services thoroughly and accurately and submit all documentation in a timely manner per agency policy and contractual/WAC requirements.
  • Demonstrate ability to work efficiently and effectively in a managed care environment, organizing workload by prioritizing and managing multiple tasks.
  • Demonstrate a working knowledge of psychotropic medications and their side effects and report any relevant observations to medical personnel.
  • Demonstrate a working knowledge of current intake and treatment approaches for clients with co-occurring disorders.
  • Demonstrate a working knowledge of the following treatment approaches and techniques: Solution Focused Therapy, Cognitive Behavioral Techniques, Stages of Change, and Dialectical Behavioral Therapy.
  • Demonstrate strong verbal and written skills. Complete and maintain all required appropriate clinical and administrative records in a timely manner, including submission of required paperwork at KMHS and community hospitals within guidelines.
  • Implement the administrative and clinical policies of KMHS as appropriate.
  • In addition to the above, any other responsibilities appropriate to the position and not specifically listed in the job description.
 
Position Requirements

Mental Health Professional (MHP) preferred: MSW or Master’s Degree in social sciences with two years of supervised relevant clinical experience. Preferred experience with in depth knowledge of the DDA population; outstanding communication abilities; collaboration and administrative skills; high degree of professional acumen; knowledge of applicable federal and state laws, rules, regulations, policies and procedures; ability to analyze and interpret findings, perform site reviews, prepare comprehensive reports and conduct public presentations. A good knowledge of community resources is helpful. Must have knowledge, skills and experience with computer programs. Must have a valid driver’s license and a reliable vehicle. Ability to travel to within Kitsap County frequently.
