Share Email Opening Title SUD Case Manager - Four Pillars Project  Description Under general supervision, the incumbent will provide case management and supportive counseling services, maintain a current knowledge of, counseling approaches, and service modalities used with individuals living with substance use disorders. The Case Manager will also provide HIV prevention counseling, testing and intervention services to individuals living with HIV or identified as high risk high risk for transmission. Maintains thorough documentation of activities including case management, crisis intervention, reimbursement, productivity statistics, prevention counseling and HIV testing. Follows agency reimbursement, regulatory and professional standards for maintaining medical records. Attends clinical staffing and unit meetings. Develops treatment plans and reviews treatment progress with contracted service providers. Participates in clinical team meetings with community providers. Provides monitoring of each client's functioning and response to planned treatment intervention.   Position Requirements Requirements: Bachelor's degree in psychology, social work, counseling or related field. Case management experience in behavioral health setting required. Good working knowledge of the disease of addiction and current AOD treatment practices.
