Description

The Community Health Educator position will primarily focus on the expansion of the new La Casa site as a Community Wellness Center. Responsibilities for this role include: serving as the Lead Health Educator in the implementation of group education activities and serving as a point of contact during working hours to all external visitors and support linkage to needed services. This position will also be responsible for implementing strategies and community events to address barriers to healthy behavioral change among the local Latino community.

Duties & Responsibilities:

  1. Coordinate activities and serve as the Lead Health Educator in the implementation of chronic disease management and health promotion interventions including: 8 week Lifestyle Enhancement Program, 6 week Diabetes Management Program and 3 week Health Promotion Program called Salud Es Conocerte. Responsilbities include:
    • Planning and scheduling weekly chronic disease management and health promotion activities at La Casa.
    • Coordinating and collaborating with promotores de salud, or community health workers in each session.
    • Track and order logistics support materials and incentives distributed in each session.
  2. Accurately track group participation data and maintain an 80% retention rate for all group interventions.
  3. Coordinate health promotion events. Coordination of events includes, but is not limited to:
    • Planning and scheduling events at La Casa Community Wellness Center and other sites.
    • Collaborating with other organizations in the development of larger health fair events.
    • Coordinating promotores de salud, or community health promoters in each event.
    • Recruiting volunteers in each event.
    • Recruiting speakers/educators as needed.
  4. Manage La Casa's room reservation system.
  5. Serve as a point of contact during working hours to all external visitors and support linkage to needed services.
  6. Build and maintain a strong personal and professional relationship with a network of community service providers.
  7. Create a referral system to connect community members to existing community services.
  8. Track, monitor and record the delivery of health education or navigation services provided using Electronic Medical Records or other data base management systems
  9. Support in the creation of culturally appropriate health education materials for dissemination in community activities, group interventions and events.
  10. Support in the recruitment, selection and training of new health promoters.
  11. Assist in the implementation of other program activities and health promoter trainings as identified by the program manager.
  12. Assist in preparing reports as required by grantors.
  13. Collaborate with other health educators within La Clinica.
  14. Participate regularly in department and program team meetings.
  15. Support supervisor in advocacy efforts with local and national coalitions
  16. Represent the Clinic with your supervisor to the public, official organizations and other groups by attending conferences, meetings, etc., as designated by your supervisor and as related to your duties
  17. Meet monthly with your supervisor to report and review your work plan.
  18. Perform other activities as identified by supervisor.
 
Position Requirements

Qualifications:

  • Associates degree in health field, or related discipline, or an equivalent combination of education and experience from which comparable knowledge and skills may be obtained.
  • Bilingual Spanish/ English; fluent spoken and written Spanish, bicultural.
  • Two years previous work experience in community health education and/or clinical services.
  • Outgoing personality and ability to relate and establish rapport with the local community, comfortable speaking in public in front of large groups.
  • Great group facilitation skills and willingness to train, mentor and build capacities of community and promotores de salud.
  • Two years’ experience working in a Latino and/or multi-ethnic communities.
  • Previous experience with community educational campaigns, including the design of educational material, charlas, and focus groups, preferred.
  • Previous experience coordinating health fairs and other health promotion events.
  • Excellent oral and written communication skills.
  • Experience in using Electronic Medical Records or similar software.
  • Proficient use of Microsoft Office and familiarity with data base management.
  • Knowledge of the Greater Washington DC metropolitan area.
