Part Time Housing Case Manager 

Description

The Richmond Behavioral Health Authority has a Part-Time Housing Case Manager to perform intermediate professional work assessing individual needs, developing, implementing and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies, counseling and assisting individuals, maintaining records and files, preparing reports, and related work as apparent or assigned.

Job Duties:

  • Provide intensive homeless outreach and case management services, to adults with serious mental illness in accordance with the HUD CoC Grant. 
  • Meet with individuals housed through RBHA on at least a monthly basis in their homes.
  • Approximately 75% of services are provided directly to individuals in the field and at offices of agency partner providers. Provide intensive outreach and screenings at shelters, city’s cold weather overflow, and lunch sites, places not meant for human habitation (streets, woods, under bridges) and other community locations to determine eligibility for Permanent Supportive Housing Program and ensure completion of all necessary forms at intake.
  • Utilize strengths perspective and positive working relationships with diverse individuals, in order to develop an ongoing effective housing focus plan with individuals.
  • Develops an individualized housing plan of care and services appropriate to the evaluation with the individual/family’s consensus; reviews and updates service plans.
  • Enter individual data into HMIS
  • Assist individuals with SMI in applying for benefits according to the SOAR Program (SSI/SSDI Outreach Access & Recovery).
  • Assesses individual service needs, capabilities and appropriateness for services; presents options and services based upon a needs assessment.
  • Identify community resources and assist individuals in accessing mainstream services; encourage individual progress and provide positive interventions for all residents.
  • Evaluates the quality of services provided and changes in housing condition and counsels individuals as necessary; evaluates individual’s environment for safety, security, negative factors and productivity; compiles and analyzes data relating to complaints; identifies and works to resolve problems.
  • Plans and facilitates group sessions with individuals participating in community activities and/or to promote recovery and educate individuals.
  • Assists individuals with activities of daily living including hygiene/bathing.
  • Orients and trains newly hired staff; evaluates staff performance as directed.
  • Serves as liaison to public agencies and provides information regarding Authority programs and services.
  • Attends inter-disciplinary team meetings to discuss decisions for individual housing plan of care; provides or arranges transportation; assists with discharge planning as needed.
  • Provides crisis intervention and prevention services.
  • Prepares a variety of reports; prepares and maintains individual charts and other records.
  • Attends meetings, staffings and conferences as they relate to individual, staff and program needs.
  • Attends court hearings; testifies in court.
  • Assists in curriculum development and resource tools.

 

 
Position Requirements

General knowledge of community and agency resources and programs of regional and state sources for the community service population; general knowledge of interviewing and supportive counseling techniques; general knowledge of social casework techniques; general knowledge of human development and behavior; general knowledge of the theories, principles and techniques of individual, family and group therapy; ability to solve problems within scope of responsibility; ability to analyze facts and exercise sound judgment in arriving at conclusions; ability to communicate complex ideas effectively, orally and in writing; ability to prepare clear and concise reports; ability to establish and maintain effective working relationships with clients, medical professionals community partners, agencies, associates and the general public.

Education and Experience

Bachelor’s degree with coursework in social work, psychology, counseling, sociology, health services or related field and moderate experience working with individuals with severe mental illness in a behavioral health setting, or equivalent combination of education and experience.

Special Requirements

Valid driver's license in the Commonwealth of Virginia.

QMHP status upon hire.

Part-time position – 20hours per week.

Starting Salary Range $20.33
