Description

PURPOSE:
Provide nursing care and medical supervision to clients in all units of 24-Hours Services (Adult Inpatient Unit, Youth Inpatient Unit, and Residential Services). Appling the nursing process to the care of all clients. Actively participate as a member of the therapeutic treatment team.

RESPONSIBILITIES AND DUTIES:

  • Examine and assess clients upon admission, determine if client is medically stable in order to provide care within the scope of practice of 24-Hours Services at KMHS.
  • Obtain admission orders from physician within one hour of admit, after obtaining pertinent information and initiate treatment orders.
  • Communicate with providers, process their orders and implement them maintaining the highest level of nursing care.
  • Document client care, medication, and treatment administration. Evaluate, monitor, and document client responses to medication, treatments, progress to mental health goals, and plan of care.
  • Write and implement nursing care plan upon admit. Review and/or revise every 24 hours. Collaborate with team ( ie: Providers, MHP's, Shift Supervisors, etc…) to establish and obtain pertinent goals.
  • Administer medications and treatment in a safe manner in accordance to standardized nursing guidelines Organize medications for administration, dispensation as directed by designated provider. Dispose of medications according to KMHS protocol. Count and order medication for clients, maintain house stock, and count and document controlled medications per shift.
  • Initiate emergency procedures using technical skills and professional judgment and perform appropriate nursing/medical intervention(s).
  • Obtain provider orders, verify and initiate, including seclusion/restraint orders per WAC 246-337-110.
  • Train, educate orient, and mentor new nursing staff, on-call nurses, and nursing students.
  • Provide nursing care including interpreting laboratory results, responses to medications, vital signs and ongoing physical assessment every shift.
  • Delegate the non-nursing tasks to support staff. Coordinate with the shift supervisor for unit safety and other client needs.
  • Participate in discharge planning, review of treatment goals, assist in client medication educational needs, ordering of discharge medications etc.
  • Participate in program activities as team member, to include but not limited to: leading wellness groups, participating with clients on the milieu, or other interactions as needed.
  • Complete nursing notes, physician orders and other documentation necessary, for quality care in an accurate, timely manner. While meeting agency, statutory, regulatory, statistical and funding requirements.
  • Respond to current fire drill/evacuation plan with awareness of duties and procedure.
  • Adhere to unit operating and nursing policies and procedures.
  • Serve as mental health professional if qualified.
  • Provide flexible nursing coverage. Rotate and be proficient in the nursing role in all units of 24-Hours Services at KMHS.
  • Disclose limited client information as necessary to the position as trained by the Records Dept. Supervisor. Disclosures must adhere to HIPAA privacy and security standards and be logged in detail according to agency policy and procedures.
  • Complete all required training and annual refreshers.
  • In addition to the above, any other responsibilities appropriate to the position and not specifically listed in the job description.
 
Position Requirements

Registered Nurse or Licensed Practical Nurse license in Washington State. Two years RN/LPN experience preferred. Two years experience in mental health preferred. Experience in child and adolescent care preferred. Experience in psychiatric environment preferred.
