Qualifications:
1.Graduate of an accredited education course in physical therapy
2.Licensed in the State of Wisconsin
3.Must not be excluded or sanctioned from State or Federal program
4.Demonstrate professional attitude and appearance
5.Proficient administrative and communication within department and facility and company
6.Thorough knowledge of the principles, methods, materials, and equipment used in the practice of their discipline
7.Display working knowledge of multidisciplinary practice scopes in the LTC setting
8.Thorough knowledge of all payer source documentation and payment requirements.
9.Ability to sue analytical skills to develop outcome driven solutions.
10.Prior EMR/computer experience
Physical Demands:
1.Must be able to walk, sit, stand, stoop, squat, bend, lift, push, pull, balance, and perform overhead activity without restriction or limitation
Duties & Responsibilities:
1.Must uphold the professional, legal and ethical standards of practice for occupational therapy as defined by the AOTA and State Practice Act
2.Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the rehab dept.
3.Performs initial patient examination, evaluation, interpretation of referrals, and creates the initial plan of care for each patient and revises as appropriate
4.Creates and maintains a patient record for each patient treats
5.Provides on-site assessment and re-evaluation of each patient’s treatment according to the State and Federal required interval, whichever is more stringent, and adjusts the treatment plan as appropriate
6.Coordinates discharge plan and decisions with the PTA
7.Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalities
8.Completes documentation according to Evergreen Rehabilitation policies including timeliness and medical necessity support.
9.Has primary responsibility for physical therapy care rendered by the PTA for all patient/clients for whom he or she is the therapist of record. In doing so:
a.Delegates appropriate portions of the treatment plan and program to the PTA consistent with the PTA’s education, training and experience
b.Must be accessible by telecommunication to the PTA at all times while the PTA is treating patients/clines for whom he or she is the therapist of record
c.Provides supervision of the PTA as required by State Practice Act
d.Provides supervisory conferences and visits as required by State Practice Act and responds to a PTAs request for additional supervisory visit for a re-examination
10.Adheres to State, Federal, and third party billing regulations
11.Participates in quality and process improvement activities in the department as outlined by manager/director
12.Partners with multi-disciplinary team to develop plan of care and advocate for patient
13.Demonstrates effective interdisciplinary communication regarding patient treatments and discharge planning
14.Cooperates with all facility and corporate departments
15.Maintains confidentiality regarding patient and company proprietary information
16.Must be able to related professionally, ,positively, and cooperatively with patients, patients family members and other employees at all levels
17.Promotes and supports the compliance program by acting consistently through Evergreen’s Code of Conduct and leading by example
18.Participates in continuous survey readiness department and facility activities
19.Ability to make effective use of available resources including time, people, materials, and financial resources
20.Ability to define problems/issues in a non-threatening, straightforward, non-accusatory manner and demonstrate ability to propose solution and execute plan

28.Asses that all rehab billing and labor entries are complete, accurate and timely

31.Demonstrates ability for continuous new learning to remain current in the field
32.Participates in clinical program development and implementation
33.Performs other duties as required and/or assigned.
