Description

The Case Manager assumes responsibility and accountability for a group of patients/clients and directs care to these patients/clients via the development of the treatment approach including specific discipline interventions. They will ensure that appropriate level of care is determined, and confer with the family referral source and the insurance company as to the status of the patient throughout treatment. The Case Manager must demonstrate excellent customer service and have enthusiasm about the program and the Pine Rest organization.

Principal Duties and Responsibilities:

  • Develops and establishes regular outreach routines in the community to connect with the homeless community and people who are at risk.
  • Engage with individuals to assess and prioritizes person’s served treatment and service needs.
  • Implements plan of care and collaborates with the clinical team to assure a uniform plan of care.
  • Provides input to the interdisciplinary treatment plan, functioning as a team member.
  • Establishes therapeutic relationship with assigned person’s served including engaging, deescalating, screening, and managing crises.
  • Serves as a liaison between community resources, family, person’s served, and hospital- or clinic-based providers of care.
  • Responsible to document and report all necessary information according to established policies and procedures.
  • Utilizes processes to conserve financial resources.
  • Creates a friendly environment for customers and person’s served by following quality improvement standards, maintaining professionalism, initiating and maintaining positive relationships with departments, co-workers and external customers.
  • Meet productivity expectations.
  • Support and promote a recovery focused environment and person-centered approach within the program.
  • Must maintain annual training and demonstrate competency in the required programmatic training.
  • Performs other duties or functions as assigned

 
Position Requirements

Required/Desired Qualifications:

Education, Training, and Experience:

  • A Master's degree in related field of study required. Must have a MSW, LPC, or LLP- can be a limited license.
  • Competency in co-occurring disorders, including ability to engage, screen, deescalate and coordinate care.
  • Two years mental health experience, to be able to independently assess person’s served needs and to implement and evaluate effective plans of care, and acquire and effectively utilize knowledge of policies and procedures.

Specific skills, knowledge, and abilities:

  • Ability to provide care/service within the scope of: Holding a conviction to express Christ’s love and compassion to recipients of services, co workers, and surrounding communities through integrity, stewardship, empowerment, the promotion of diversity, and professional excellence.
  • Ability to provide cultural competent care with a wide range of individuals in a diverse community setting.
  • Interpersonal skills necessary in order to communicate effectively with person’s served, families, community partners and employees.
  • Analytical skills necessary in order to make clinical judgments and assessments, develop treatment plans, design and evaluate effectiveness of therapeutic activities, and monitor person’s served progress.
  • Requires physical and emotional stamina to deal with physical, emotional, social, and spiritual needs of person’s served.
  • The candidate may be required to have a valid Michigan driver’s license and ability access to access Pine Rest Transportation Vehicles for the performance of job duties.

About the Organization Pine Rest is a nonprofit, free standing, behavioral health system founded in 1910 in Grand Rapids, Michigan. This comprehensive behavioral health center, coupled with the Pine Rest network of 19 outpatient clinics and satellite offices in Michigan and Iowa, provide individualized and group therapy for people of all ages and all levels of need. Pine Rest clinicians offer therapeutic services in behavioral health for people of all ages and all levels of need across the continuum of care within a Christian framework.

Pine Rest has a 200-bed inpatient hospital program with a 36-bed child and adolescent inpatient and a large partial hospital program for adolescents. Additionally, Pine Rest has many inpatient services for adults, including: a 28-bed psychiatric-medical unit in an acute care hospital, four 22-bed adult inpatient units each designed for a different level of acuity, a 22-bed co-occurring substance use disorder adult inpatient unit, a large adult partial hospital program, and a 26-bed older adult inpatient unit.

The Pine Rest Community and Residential Services Division also provides a wide array of services. With over 220 residential beds across a variety of programs, including treatment for substance use disorders, homes for adults with intellectual and developmental disorders, adults with mental illness, and a program for youth aged 11-18 with extensive trauma histories focusing on trauma treatment and life skills programming.

Other campus based services include a forensic psychiatry clinic, a psychological consultation testing center, an attention deficit disorders clinic, an autism spectrum disorders clinic, and a large neuromodulation clinic which provides TMS and ECT services.

The medical staff includes over 70 psychiatrists and other MDs/DOs, more than 120 fully licensed psychologists, more than 250 licensed masters-level social workers, and 30 PAs/NPs.
