Provides leadership to an interdisciplinary Care Team which addresses the comprehensive behavioral health needs, and coordinates the physical health needs, of individuals suffering from severe or chronic mental illness and/or those with co-occurring disorders. This team works closely with Jail staff and the courts to Ensures high quality treatment for incarcerated and/or court involved individuals, within a recovery framework, to improve functioning and health. Supports clinicians in the performance of their duties through oversight and participation in clinical coordination and problem solving within the Care Team, and with clients, other agency programs and community resources. Monitors and ensures compliance with all applicable legal, regulatory and contractual requirements. Provides regular review of assigned clients, including their clinical records, to ensure appropriate service delivery, community linkages, and documentation.
The Jail Diversion Team works within a network of internal and external client service teams to ensure an integrated approach to providing mental health, chemical dependency, and physical health services and solutions. This position involves significant program development and a high level of fluency in web-based electronic health records, community resources in Kitsap county and complex health issues.


  • Direct the day-to-day clinical operations of the care team including staff work hours to assure appropriate coverage. Lead the daily organizational or treatment planning meetings. Continuously evaluate the status of clients and do appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs. Monitor quality of clinical work done by team members. Provide training and mentoring for supervisees
  • Develop and implement an integrated treatment model for clients with dual disorders. Facilitate programmatic integration in a multi-disciplinary context.
  • Ensure clients have clearly delineated treatment goals, crisis and discharge plans that address needed behavioral health interventions and physical health care coordination. Ensure systems are in place to efficiently and consistently communicate treatment goals and interventions to all staff. During staff turnover of staff or transfer of clients ensure seamless continuity of care, at minimum risk for clients.
  • Participate in staff recruitment, interviewing, hiring, work assignments, and orientation and performance supervision according regulations, and agency policies and procedures. Develop and implement staff orientation and training.
  • Assist clinicians in the performance of their duties by monitoring and participating in clinical coordination/problem solving between clients and clinicians and other agency programs and community resources.
  • Work closely with Health Analyst to monitor health status and medical services utilization of clients assigned to the Care Team and ensure follow up and transitional care planning as needed.
  • With Co-Occurring Disorder and Chronic Disease/Wellness Trainers, continually assesses knowledge and skill needs of Care Team members and advise regarding training and curricula development.
  • Participate in program development and management tasks. Operate as a member of the management team. Work with other supervisors and unit managers to coordinate across programs to ensure continuity of clinical care, including assessing and communicating program needs, formulating on-going program goals, and participate in quality program reviews. Help develop and implement agency policies and participate in problem solving regarding clinical and fiscal issues.
  • Carry out and document quality-assurance activities and reviews of the use of program services.
  • Initiate and maintain effective and on going communications with other human services organizations, law-enforcement, and other community resources (e.g. landlords, employers), and with other agency departments and programs including the Crisis Response Team, the Adult Inpatient Unit, and Residential Services. Work closely with clients' families, advocacy groups, and ombuds services to ensure an open and receptive communication process.
  • Maintain a safe and secure working environment.
  • Facilitate professional growth of all clinicians.
  • Monitor and make program revisions as necessary to improve the continuity of care and quality of services to ensure customer satisfaction with treatment and clinical effectiveness.
  • Participate in cross-system committees appropriate to assigned work to help further the effectiveness of the services that KMHS provides.
  • In addition to the above, any other responsibilities appropriate to the position and not specifically listed in the job description.
Position Requirements

Position requires a master’s degree in nursing, social work, psychiatric rehabilitation, psychology, or a related field and must meet the definition of a Mental Health Professional. Experience providing COD treatment preferred. Experience working in the Jail system and with the Courts preferred. A combination of exceptional clinical and organizational skills is a must, as is experience working with adults with severe and persistent mental illness. Supervisory and program management experience is desired. A valid Washington State driver’s license and business-rated insurance are required.
