Description

PURPOSE: Provide support and assistance to the Care Team staff and clients so as to ensure timely and targeted comprehensive care interventions and coordination. Facilitate appropriate and timely preventative, specialty, and transitional health services to Care Team clients. Ensure client health data is accurately tracked in the client health record.

RESPONSIBILITIES AND DUTIES:

  • Obtain and record vital signs, administer injections, complete lab orders, perform venipuncture and CLIA waived lab tests, and track lab results as requested by psychiatric medical provider.
  • Submit and monitor status of necessary prior authorizations and assist with medication refills.
  • Utilize KMHS approved external resources to obtain progress notes and diagnoses, view laboratory results, enter data into National registries, verify insurance coverage information and monitor prescriptions for controlled substances in the state data base.
  • Provide support for Care Team Supervisor and staff to interpret, prioritize and present client health data to the care team. Communicate key client health needs to the care team members for comprehensive care coordination and treatment planning.
  • Assist with gathering and tracking health information and accurately enter data into the electronic medical record.
  • Assist the care team with timely communication with primary and specialty medical care providers, acute care hospitals, inpatient psychiatric treatment facilities and emergency departments. Establish effective linkages with on-site PCP and community medical entities to coordinate client needs.
  • Participate with other care team members in the development of client treatment plans and the coordination and facilitation of client care.
  • Provide information and education to clients to support their understanding of health and self-management capabilities.
  • Stay abreast of health related practices and make recommendations regarding process and content improvements.
 
Position Requirements

MINIMUM QUALIFICATIONS: Current Medical Assistant-Certified (MA-C) credential with Washington State Department of Health. Ability to work within interdisciplinary teams on behalf of complex clients with extensive behavioral care needs. Experience with EMR and data tracking and ability to read data reports. Experience with venipuncture and intramuscular injections. Familiarity with outpatient medical clinic operations so as to facilitate the establishment of positive and productive relationships with medical practice staff throughout the community. Effective collaboration and communication skills. AAC (Agency Affiliated Counselor) certification with WA State required within 60 days of hire.

HYSICAL CONSIDERATIONS: While performing the duties of the job, the employee is required to stand, walk, sit, use hands and fingers, talk or listen. Requires the ability to lift up to 10 pounds. This position also requires close vision and peripheral vision.
