Description

Catholic Charities, Diocese of Trenton is one of only 7 behavioral health providers selected to partner with the State of New Jersey on the development of Certified Community Behavioral Healthcare Clinics. Join our team and be part of this exciting innovation!

The Director of Integrated Care is a dynamic opportunity to lead a federal initiative for integrated care and population health. This Director position will develop and supervise the CCBHC initiative in Mercer County and will provide strategic direction on integrated models of care.

Essential Functions:

  • Operationalize and manage CCBHC model and develop care pathways
  • Develop and implement protocols for interdisciplinary treatment teams
  • Develop Evidenced Based Practice training continuum for CCBHC staff and monitor competency
  • Manage relationships with partner organizations
  • Ensure data aggregation and reporting requirements are met
  • Operationalize utilization review, quality assurance and compliance functions
  • Collaborate with agency leadership and treatment teams for outcomes management.
  • Supervise CCBHC staff and ensure that budget parameters are met
  • Maximize use of technology use for consumer engagement, staff productivity, measures output, clinical decision supports and billing systems

 
Position Requirements

Master’s degree in Social Work or Professional Counseling with licensure preferred. Must have a minimum of five years supervisory experience and a minimum of two years of experience working in integrated services programs or programs incorporating population health initiatives.
