Description

Horizon Healthcare, Inc., sister company of Genesis Behavioral Services and Matt Talbot Recovery Services, is seeking a regular, Full-Time Transitional Outreach Specialist at Oshkosh Correctional Institution (1730 W Snell Rd., Oshkosh, WI 54903).

Duties & Responsibilities

  • Act as an advocate for securing services needed by newly-released correctional offenders
  • Work with program participants during the last six (6) months of incarceration and first twelve (12) months of community re-entry
  • Review each participant's Community Reintegration Offender Questionnaire (DOC - 2266) no less than 90 days before release
  • Assist participants in completing the competencies listed in the Standardized Pre-Release "Personal Development" module prior to community re-entry. (Additional readings and/or assignments pertaining to coping skills may be subsequent given the needs of individual participants)
  • Inform and update program participants of service providers and initial contacts located in the community they are returning to
  • Research employment, vocational training, and pro-social support groups and share with program participant
  • Schedule and coordinate home visits with participants at least monthly the first four (4) months following their release and bi-monthly the remaining eight (8) months their first year in the community
  • Identify the medical and/or psychological needs of participants in order to assess their aftercare and reintegration needs and the availability of community-based resources to meet these needs
  • Consult with DAI social worker and agent to develop an aftercare and reintegration plan, addressing risk and ongoing treatment needs, including: level of supervision, residential placement, income sources, clinical service needs, treatment programming, vocational training and employment
  • In consultation with DAI agent and Unit Manager, implement, reassess, and revise aftercare case plans based on the participants' performance, progress, current treatment needs, and availability of aftercare resources
  • Construct an automated database to collect, analyze, and generate data necessary to support evaluation of the program

Position Requirements

Qualifications & Requirements

  • A Criminal Background Check may be required for this position subject to Wisconsin law
  • High School Diploma or equivalent
  • Associate Degree in Human Services, OR
  • Post-High School experience with successful outreach services to offenders
  • Minimum two (2) years’ experience with outreach services in a Human Services environment
  • Valid Wisconsin Driver License with no at-fault accidents within the last two (2) years
  • Current auto insurance and dependable vehicle for work-travel-related purposes
