Description

Candidates will be contacted to fill in on vacant shifts as needed, will work in various IDD group home settings in Hampton and Newport News.  Must be willing to work weekends, evening and overnight on a consistent basis.  Must remain current with all training and employment requirements. 

CLASS SUMMARY                                                                                              

The Community Services Associate II is responsible for providing direct care in the private home or residential facility; in conjunction with assisting the Program Supervisor in providing direct service treatment and case coordination for adults with Intellectual and Developmental Disabilities in an Intermediate Care Facility.

DISTINGUISHING ELEMENTS OF THE CLASS:

I. IMPACT:  Immediate health and welfare of client is directly affected by work of this class.  Activities of incumbent can promote successful community integration and rehabilitation for the client and assist in preventing unnecessary institutionalization.

II. LATITUDE:  Day-to-day work is performed independently with regular consultation and frequent follow-up from the QMRP and/or Program Supervisor.  Work is guided by the Individual Service Plan, ICF/IDD regulations and guidelines, active treatment schedules and Program Schedule.

III. RESPONSIBILITY FOR THE WORK OF OTHERS: Class supervises no staff; however, supervision is exercised as Shift Leader during work shifts.

IV. INTERPERSONAL CONTACTS:  Continuous contact with clients and other staff in planning and providing client services.  Regular contact with family members and members of other agencies providing services to clients, such as social workers, employers, physicians and other service providers.

V. DIFFICULTY:  Appropriately relating to a variety of client personality types requires use of judgment and discretion in emergency situations resulting from client medical or behavior problems involve timely and responsible decision making.  Incumbent must deal with a variety of other human services officials implementing individual program plan delivery.

VI. COMMUNICATIONS:  Work involves regular oral and written communication with Program Supervisor, QMRP, clients, employers, and families.  Work also requires written documentation such as reports, quarterly/annual reviews, business communications, client data collection, etc.

VII. EQUIPMENT/FACILITIES RESPONSIBILITY: Incumbent will be responsible for equipment at residential sites such as vehicles, adaptive equipment, TV’s, Microwave ovens, stoves, dishwashers, recreation equipment and associated furnishings.

VIII. WORKING CONDITIONS:  Work is performed in residential sites. Incumbent works at the facility or site during assigned shift.  Frequent outside activities expose incumbent to elements of nature.  Some clients can be abusive.  Work hours often include evenings, overnights and weekends.  Supervisor may not be on site during these times.

IX. KNOWLEDGE, SKILL AND ABILITIES (KSA'S):

Good knowledge of mental retardation, autism, and other developmental disabilities.

Demonstrated skill in working with adults with disabilities.

Very good oral and written communication skills.

Ability to establish and maintain effective working relationships with clients, families, employers and other service provider staff.

Ability to ascertain ICF/IDD regulations.

Ability to follow active treatment plans and schedules.

X. LICENSURE:

Valid VA driver's license.

CPR Certification*

CPI Training

Medication Training (as required by program)

First Aide Certification*

Valid Virginia CNA license (if applicable)

Food Handler Certification

* May be obtained within first 30 days of employment.

XI. PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Obtain, develop and create training materials and/or activities necessary to enable clients to reach their goals.

Monitor special medical conditions, i.e. seizures, diabetes, special diets, physical disabilities and dual diagnosis.

Serve on committees relevant to the needs of the program and/or participants, i.e. safety committee, incentive committee, decorating committee to improve the overall appearance of the facility, planning committee for staff activities.

Recommend revisions to be made on functional skill goals to better serve the client’s specific needs.

Responsible for the collection and dispersement of funds for clients.

Maintain accounting records and disperse receipts to appropriate departments.

Builds a positive and realistic image of all programs within the community.

Maintains client documentation as required by program standards, i.e. Medicaid, ICF/IDD regulations, licensure, social services, CARF, program policies and procedures, etc.

Maintain orderly confidential files.

Ensure client’s personal hygiene by performing bathing needs.

Ensure meal preparation and/or feeding of clients as directed by physician orders.

Assist and perform household services as are essential to client care and/or facility upkeep.

Interact appropriately with clients, families, employers and other service provider staff.

Report any difficulties/problems to the designated Supervisor of the appropriate program.

Safeguard the health, safety and civil and personal rights of all clients/families.

Safeguard the confidentiality of clients and records pertaining to clients.

Dispense medication.

Perform other job related duties as assigned by Supervisor.

Represent program on committees and attend various meetings as requested by supervisor/QMRP.

Interface with other human services officials engaged in providing services to client (social workers, therapists, physicians).

Conduct training and assistance as related to individual program plan.

Complete incident reports as necessary.

Conduct fire drills when required.

Promote active treatment continuously.

Collect data of training objectives.

Transports clients as necessary.

XII. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. Continuous monitoring of clients and some running after clients, lifting from 100-300 lbs, walking up and down stairs, pushing and pulling wheelchairs 275-350 lbs, assisting clients on and off vans, and some kneeling and crawling. Use of Holier Lifts and other adaptive devices. Must assist with direct client care, to include: bathing, feeding, assisting with toileting and diaper changing, handling non-ambulatory consumers and their assistive technology such as wheelchairs and walkers and managing a wide range of difficult and aggressive behaviors.
