Description

CLASS SUMMARY:

The Community Services Associate II is responsible for providing direct care and supervision of individuals who are seriously mentally ill and/or co-occurring substance use disorder in an intensive supervised residential home toward maintaining community placement; in conjunction with assisting the Program Supervisor in providing direct service treatment and case coordination.  Monitors and documents client progress.

DISTINGUISHING ELEMENTS OF THE CLASS:

I.  IMPACT:  Immediate health and welfare of client is directly affected by work of this class.  Activities of incumbent can promote successful community integration and rehabilitation for the client and assist in preventing unnecessary hospitalization.

II.  LATITUDE:  Day-to-day work is performed independently with regular consultation and frequent follow-up from the QMHP and/or Program Supervisor.  Work is guided by the Individual Service Plan, Medicaid & Licensure regulations and guidelines, active treatment schedules and Program Schedule.

III.  RESPONSIBILITY FOR THE WORK OF OTHERS:  Class supervises no staff.

IV.  INTERPERSONAL CONTACTS:  Continuous contact with clients and other staff in planning and providing client services.  Regular contact with family members and members of other agencies providing services to clients, such as social workers, employers, physicians and other service providers.

V.  DIFFICULTY:  Appropriately relating to a variety of client personality types requires use of judgment and discretion in emergency situations resulting from client medical or behavior problems involve timely and responsible decision making.  Incumbent must deal with a variety of other human services officials implementing individual program plan delivery.

VI.  COMMUNICATIONS:  Work involves regular oral and written communication with Program Supervisor, QMHP, clients, employers, and families.  Work also requires written documentation such as reports, quarterly/annual reviews, business communications, client data collection, etc.

VII.  EQUIPMENT/FACILITIES RESPONSIBILITY:  Incumbent will be responsible for the safe use and security of tools, equipment, appliances, recreation equipment and associated furnishings, and space utilized by the clients in the course of supervised skills training, at residential site.

VIII.  WORKING CONDITIONS:  Work is performed in a supervised residential site.  Incumbent is exposed to hostile, aggressive, and verbally abusive clients presenting a risk of personal harm.  Outside activities expose incumbent to elements of nature.  Work hours include overnights and weekends.  Supervisor may not be on site during these times.

IX.  KNOWLEDGE, SKILL AND ABILITIES (KSA'S):

Good knowledge of serious mental health disorders, and of the principles and practices relating to the care of individuals who have these disorders.

Demonstrated skill in working with adults with disabilities.

Ability to communicate effectively both orally and in writing.

Ability to establish and maintain effective working relationships with clients, families, employers and other service provider staff.

Ability to evaluate and interpret behavioral observations.

Ability to follow active treatment schedules.

Knowledge of federal and state local support services and laws governing client’s rights and confidentiality of records.

Must be proficient in the use of computers.

X.  LICENSURE:  Valid Virginia State Driver’s License

XI.  PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Conducts individual and group development training such as independent living skills, recreation and leisure activities.

Monitor special medical conditions, i.e. seizures, diabetes, special diets, physical disabilities and dual diagnosis.

Recommend revisions to be made on functional skill goals to better serve the client’s specific needs.

Maintains client documentation and other documentation as required by program standards, i.e. Medicaid, licensure, CSB & program policies and procedures, etc.

Maintain orderly confidential files.

Assist and perform household services as are essential to client care and/or facility upkeep.

Interact appropriately with clients, families, employers and other service provider staff.

Report any difficulties/problems to the designated Supervisor of the appropriate program.

Safeguard the health, safety and civil and personal rights of all clients/families.

Safeguard the confidentiality of clients and records pertaining to clients.

Dispense medication.

Perform other job related duties as assigned by Supervisor.

Transports clients as necessary.

XII.  PHYSICAL REQUIREMENTS:  Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls.  Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine.  Walking, moving and carrying boxes, office supplies, usual climbing, bending, kneeling, crawling, reaching, handling, sitting, standing, pushing, and pulling.
