Description

CLASS SUMMARY:

In conjunction with the Supervisor of Residential Services, is responsible for providing oversight to direct support staff during designated shift and for providing direct service, treatment, and case management for adults who have intellectual disabilities and who may have other disabilities including physical, cognitive, or mental disorders. Position is responsible for reviewing and maintaining documentation/records, ensuring active treatment are delivered (ICF), ensuring billable services, implementing person centered plans (PCP), activity coordination, medication administration, and assisting with the management of staffing coverage.

DISTINGUISHING ELEMENTS OF THE CLASS:

I.  IMPACT:    Immediate health and welfare of individuals receiving services is directly affected by work of this class.  Activities of incumbent can promote successful community integration for the individuals and assist in preventing unnecessary institutionalization.  Maintains confidential files and attendance/billing records to ensure services are provided and billed in accordance with Medicaid and Department of Behavioral and Developmental Services (DBHDS) guidelines.  Billing of services is crucial to program operations; failure to understand or comply with guidelines could result in non-payment. Is responsible for ensuring compliance and ongoing adherence to person-centered practices.

II.  LATITUDE:    Day-to-day work is performed independently with regular consultation and frequent follow-up from the Residential Supervisor.  Work is guided by the PCP/ Individual Service Plan (ISP), program policies, guidelines, and instructions.

III.  RESPONSIBILITY FOR THE WORK OF OTHERS:    Oversees work of direct support staff during designated shift. Provides instruction and feedback to staff during assigned shift.

IV.  INTERPERSONAL CONTACTS:    Continuous contact with individuals receiving services and program staff in planning and providing services.  Regular contact with family members and members of other agencies providing services to individuals, such as social workers, physicians, and case managers.

V.  DIFFICULTY:    Appropriately relating to a variety of individuals’ personality types requires use of judgment and discretion in emergency situations resulting from individuals’ medical or behavior problems involving timely and responsible decision making.  Incumbent must deal with a variety of other human services officials implementing services plan delivery.  Incumbent must be constantly aware of all Medicaid billing reports and knowledgeable of reimbursement rates, guidelines and procedures and respond to emergency staff leave requests.  Incumbent must be able to effectively delegate shift assignments and manage difficult situations that may arise such as staff call-outs and medical emergencies.

VI.  COMMUNICATIONS:    Work involves regular oral and written communication with Supervisor, Case Managers, Program Administrators, Program Nurse, Medication Supervisor, Staff Development & Training Coordinator, Behavior Specialist, Doctors, Fiscal/Reimbursement Administrators, Medicaid Transportation providers, individuals, and families.  Work also requires documentation in the agency’s electronic health records, as well as written documentation such as business communications and financial reports, etc.  Public relations activities are frequently involved.  Contact with other clinicians requires tactful approach to ensure clinician cooperation.

VII.  EQUIPMENT/FACILITIES RESPONSIBILITY:   Responsible for assisting Supervisor with the maintenance and security of the residence and associated furnishings and supplies.

VIII.  WORKING CONDITIONS:  Work is performed in a residential environment.  Incumbent works at the residence during assigned shift.  Occasional overnight may be required.  Frequent outside activities expose incumbent to elements of nature.  Some individuals can be abusive.

IX.  KNOWLEDGE, SKILLS AND ABILITIES (KSA’S):

Good knowledge of intellectual and developmental disabilities, autism, and other disabilities

Demonstrated skill in working with adults with disabilities

Very good oral and written communication skills

Ability to establish and maintain effective working relationships with superiors, individuals, external providers, and direct support staff

Experience with utilization of electronic records

Good knowledge of licensure, accrediting agencies, and clinical documentation requirements

Good knowledge of the internal relationships of various parts of the clinical record

Ability to recognize documentation error trends and inform supervisor

Good knowledge of Medicaid programs and billing process

Ability to assist in a variety of administrative assignments simultaneously

Good knowledge of medication administration

Good time management and organizational skills

X.  LICENSURE: Valid Virginia Driver’s License.

XI.  PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Provide oversight to direct support staff on designated shift

Ensure that all necessary duties are completed during designated shift

Instruct direct support staff and model proper procedure for implementing individuals’ PCPs/ISPs

Review program documentation and activities to ensure adherence to PCPs and person-centered practices, and reports deficiencies to Residential Supervisor

Inform Supervisor of trends in clinical documentation

Complete incident reports as necessary and ensure that incident reports are completed on assigned shift

Coordinate individualized activities on designated shift and ensures that activities are completed

Ensure proper shift change and documentation/notification of problems noted on shift

Review plan checklists to ensure justification for program billing

Report all billing problems or discrepancies to the Residential Supervisor

Review and maintain medication logs and necessary documentation

Administer medications and perform other direct care duties as needed

Maintain accurate documentation of individuals’ attendance

Maintain inventory records of facility supplies

Serves as lead staff member in absence of Residential Supervisor and is the primary communicator with Residential Supervisor during designated shift

Assist Supervisor with scheduling individuals’ plan reviews and maintenance of confidential files

Assist in staff development activities and documentation training

Represent program on committees and attend various meetings as requested

Interface with other human service officials engaged in providing services to individuals on their caseload (social workers, case managers, physicians, etc.)

Build a positive and realistic image of all programs within the community

In conjunction with Supervisor, conducts assessments and suggests revisions to PCP/ISP

In conjunction with Supervisor, maintains clinical documentation for compliance with program standards and regulatory entities (i.e. Medicaid, DBHDS, Department of Health, HUD, etc.)

May assist with development of staff schedule and assist with finding staff coverage

May maintain petty cash fund and appropriate documentation as required by Fiscal

May coordinate staff training schedule and keep records of all training attended

Performs other duties as assigned by Residential Supervisor

Transports clients as necessary.

XII.  PHYSICAL REQUIREMENTS:    Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls.  Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a:  personal computer, calculator, copier, and fax machine.  Some walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, handling, sitting, standing, pushing, and pulling. Some running after individuals, walking up and down stairs, assisting individuals on and off vans, kneeling and crawling, some lifting (with a Hoyer lift and staff assistance) from 100-200 pounds, pushing and pulling individuals in wheelchairs with a total weight of 150-250 pounds.
