Description

CLASS SUMMARY

The Community Services Associate II is responsible for providing direct care in the private home, residential facility, center based and community based sites; in conjunction with assisting the Program Supervisor in providing direct service treatment and case coordination for adults with Intellectual and Developmental Disabilities.

PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

  • Obtain, develop and create training materials and/or activities necessary to enable clients to reach their goals.
  • Monitor special medical conditions, i.e. seizures, diabetes, special diets, physical disabilities and dual diagnosis.
  • Serve on committees relevant to the needs of the program and/or participants, i.e. safety committee, incentive committee, decorating committee to improve the overall appearance of the facility, planning committee for staff activities.
  • Recommend revisions to be made on treatment goals to better serve the client’s specific needs.
  • Responsible for the collection and dispersement of funds for clients.
  • Maintain accounting records and disperse receipts to parents and guardians.
  • Builds a positive and realistic image of all programs within the community.
  • Maintains client documentation as required by program standards, i.e. Medicaid, licensure, social services, CARF, program policies and procedures, etc.
  • Maintain orderly confidential files.
  • Ensure client’s personal Hygiene.
  • Ensure meal preparation and/or feeding of clients as directed by supervisor or family.
  • Assist and perform household services as are essential to client care and/or facility upkeep.
  • Interact appropriately with clients, families, employers and other service provider staff.
  • Report any difficulties/problems to the designated Supervisor of the appropriate program.
  • Safeguard the health, safety and civil and personal rights of all clients/families.
  • Safeguard the confidentiality of clients and records pertaining to clients.
  • Dispense medication.
  • Perform other job related duties as assigned by Supervisor.
  • Represent program on committees and attend various meetings as requested by supervisor.
  • Interface with other human services officials engaged in providing services to client (social workers, case managers, physicians).
  • Provide training and assistance as related to individual client service plan.
  • Complete incident reports as necessary.
  • Conduct fire drills when required.
  • Transports clients as necessary.
