Description

CLASS SUMMARY:

Responsible for performing clinical and coordination services as a part of HPR-V Virginia REACH Program.  Position provides direct one-to-one care as well as therapeutic group counseling and services to individuals with IDD and co-occurring mental or behavioral health disorders who are enrolled in the Region V VA REACH Program, receiving temporary crisis intervention services in the crisis therapeutic home (CTH).  The incumbent will assist the individual with meeting treatment goals, scheduled daily activities, ADL’s, and providing de-escalation and intervention services for behavioral concerns.  Services will be provided in the HPR-V VA REACH CTH and/or in the individual’s natural living environment.  The incumbent must possess the ability to communicate effectively and work collaboratively with individuals receiving services, their guardians, support system, and the VA REACH Program regarding any significant events, the individual’s needs and their response to the intervention.  Monitors daily activities and provides documentation regarding the progress of individuals in care.

PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

  • Plans and conducts individual and group development training such as independent living skills, recreation and leisure activities.
  • Psychiatric, neuropsychiatry, and psychological assessment, and other assessment and stabilization techniques.
  • Provides individual and group supportive counseling.
  • Performs behavioral assessments and implements positive behavior support.
  • Advocates for individual and general individual rights and services.
  • Monitors and documents progress and prepares individual records.
  • Schedules and coordinates appointments.
  • Interfaces with and supports, individuals’ family members and/or care givers.
  • Intensive care coordination with other agencies and providers to assist with the delivery of services.
  • Medication management and monitoring.
  • Training of family members and other caregivers and service providers in positive behavioral supports to maintain the individual in the community.
  • Temporary Crisis Supervision.
  • Other duties as required.
  • Transports individuals as necessary.  Incumbent may transport individuals or guest utilizing the agency vehicle and/or personal vehicle.
