JOB SUMMARY: This position is responsible for the coordination and delivery of substance abuse counseling designed to meet the wellness and recovery needs of the consumer. Depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumer’s family members. ESSENTIAL FUNCTIONS Intake: Conduct initial and ongoing assessments utilizing the NJSAMS/Anasazi Clinical System. Develop a treatment plan focusing on target/problem areas and measurable objectives that have been mutually identified by consumer and clinician. Clinical Services: Meet individually with clients on a weekly basis to monitor progress toward identified objectives. Utilize structured curricula and protocols to promote psycho-education, skill acquisition and to maintain focus in groups. Involve and engage family and significant others in the recovery process and assists them in understanding the illness and managing its effects. Maintain communication with clients, including outreach for hard to engage clients. Develop formal aftercare plans for clients completing a particular stage of treatment. Prepare a discharge summary for consumers whose case is being closed. Documentation: Documentation should be maintained in Anasazi, the agency’s electronic health record software, and should include: the assessment, treatment plan, weekly progress notes of all direct and indirect activity, collateral contacts, and termination summary. Case Management: Establishes and maintains communication with referral sources and other agencies and treatment providers. Identifies and accesses community resources to support group activities. Consults with other service providers and documents their participation in the recovery process. Maintains accurate and up to date case records consistent with agency and funding source requirements. Productivity: Clinicians are expected to spend 60% of their time in face to face consumer contact and to provide routine productivity reports to their supervisor. All schedules, statistical reports/logs, and clinical reports are to be submitted within proscribed timelines. Documentation should be completed in Anasazi within 24 hours of service delivery. Supervision: Clinicians are required to participate in individual and peer supervision according to their program needs. OTHER DUTIES Participate in the Quality Improvement process and specific quality initiatives for the program. Interface with medical, vocational, residential and other program resources as necessary. Utilize supervision and training as opportunities for enhancing professional competence. Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, and professional standards. Other related duties as assigned. |
