REACH - Regional, Education, Assessment, Crisis Services, Habilitation

REACH is a 24/7 mobile crisis intervention program that provides crisis support to adults with Intellectual and Developmental Disabilities.  The goal of the program is to prevent unnecessary institutionalization of individuals with Intellectual and Developmental Disabilities and to provide them opportunities to live in the most integrated setting appropriate to their needs and consistent with their informed choice. REACH provides in-home and community based crisis assistance services to individuals and their families in order to improve the quality of life and decrease facility and hospitalization placements. This class provides direct short term crisis residential services for individuals served with intellectual and developmental and behavioral health disabilities. Supervises full and part time staff engaged in residential care.  Responsible for the overall program management of planned and unplanned residential respite component of REACH services.

CLASS SUMMARY:

The position is responsible for administration, management, and clinical oversight of the HPR V REACH program. Essential Job Functions include the overall responsibility for the implementation and continued success of the REACH Crisis Services for individuals with intellectual and developmental disabilities and co-occurring behavioral needs to include emergency respite, planned respite and in-home supports and services.  Ensures that assigned treatment programs are responsive, efficient and congruent with the CSB’s clinical and fiscal goals and objectives. Provides clinical direction and consultation to assigned program clinical team, staff and region.  Develops, manages and monitors program budgets, completes necessary administrative supports, develops, monitors, and assesses productivity and outcomes data.  Develops and implements marketing and customer relations strategies to promote high utilization of clinical programs and promotes new business opportunities. Provides clinical and administrative leadership to ensure that clinical services are integrated across all disciplines and divisions.  Ensures that all clinical programs are in compliance with applicable regulatory/licensure/accreditation standards.

DISTINGUISHING ELEMENTS OF THE CLASS:

I.          IMPACT:  The health and welfare of clients are directly dependent on the work of this class.  The incumbent’s work affects the availability and quality of clinical services and supports available to our customers.  Incumbent is responsible for the health and safety of the consumers, their families and others served in assigned programs.  This position represents the agency in various external settings and directly influences public opinion of the CSB.  This position has responsibility for ensuring that services are accessible, responsive, coordinated, sufficient and efficient.  This position ensures that all assigned programs meet or exceed budgetary expectations.  The position is directly accountable for the successes or failures of assigned programs.  The work of the incumbent is highly visible to consumers, regulatory agencies, the public, other service providers and public agencies.

II.        LATITUDE:  Work is performed independently with consultation from the Senior Director.  Clinical work is guided by the standards and practices of the profession and substantial discretion and independent judgement is exercised in clinical matters.  Administrative work is guided by agency policy, state and federal regulations, and sound business practices.  Regular consultation on productivity, outcomes measurement, and financial operations and conditions  is provided by the Director of Financial Services.

III.       RESPONSIBILITY FOR THE WORK OF OTHERS:  Position directly supervises the  REACH Clinical Team and Respite  Home Supervisor.  The incumbent recommends appointment and termination to the Senior Director.  Provides leadership, supervision and education to assigned staff.  Provides consultation to Regional ID Directors and Management Team.

IV.       INTERPERSONAL CONTACTS:  Work involves regular contact with Case Managers Emergency Services, ID Directors, Regional Reinvestment Project Director, Executive and Division Directors, Program Managers, Clinical and Residential Supervisors, Clinicians, Psychiatrists, clients, client families, private providers, local, state and federal assistance agencies, law enforcement, courts, and criminal justice officials, legal representatives, civic and community groups, governmental licensing personnel, housing authority personnel, health care providers and institutions, managed care organizations and other insurers.

V.        DIFFICULTY:   Work involves the application of highly complex diagnostic and clinical skills to a seriously impaired consumer population presenting a wide range of diagnoses.  The incumbent establishes policies, procedures and protocols that guide the development, implementation, and evaluation of treatment programs and services.  The incumbent provides leadership to the staffs of multiple programs and directly supervises the treatment efforts of other clinicians.  This position’s professional duties, and consultative responsibilities require  the incumbent to maintain competency in a dynamic and highly technical field.  Work involves program planning, marketing, budget development and management and exercise of effective leadership. Maintain compliance with strict Medicaid and Intermediate Care Facility  requirements and state and local regulations.

VI.       COMMUNICATIONS:  Work involves the preparation and presentation of detailed funding, grant and other program proposals including public presentations to current and potential client families, and other professionals; program policies, procedures and protocols, and treatment plans and reports.  Public speaking and representing the agency to service providers, and the public will frequently be required.  Work requires a high degree of oral and written communication skills.

VII.     EQUIPMENT/FACILITIES RESPONSIBILITY:  Incumbent operates a motor vehicle and various other office and computer equipment.

VIII.    WORKING CONDITIONS: Incumbent works in an office environment, client homes and treatment and residential facilities.  Work requires travels to multiple area sites.

IX.       KNOWLEDGES, SKILLS AND ABILITIES (KSA'S):

Thorough knowledge of the diagnosis and treatment of the ID population

Thorough knowledge of the principles and practices related to the operation of a residential facility for persons who have intellectual and/or other disabilities.

Thorough knowledge of treatment theories, principals and techniques pertaining to common disabilities relating to the care and behavioral management of those with  intellectual and developmental disabilities.

Thorough knowledge of Person Centered Planning.

Thorough knowledge of the laws and legal procedures relating to the provision of ID and MH services.

Thorough knowledge of Intellectual disabilities, autism, and other developmental disabilities, physical, emotional, and sensory disorders, and  the operation of the designated  programs principles and operations.

Thorough knowledge of Medicaid Waiver and Licensure regulations.

Excellent leadership, organizational and training skills.

Excellent oral and written communications skills, public speaking skills and negotiation skills.

Ability to develop, monitor and implement complex budgets.

Ability to plan, supervise, evaluate and coordinate the work of clinical and administrative professionals.

Ability to establish effective working relationships with other agencies, family members and regulatory bodies such as DBHDS, and DMAS.

Ability to collect, compile and evaluate statistical and financial data and to prepare complex written reports and records.

Ability to work independently.

X.        LICENSURE: Virginia licensed as a LCSW (Licensed Clinical Social Worker), LPC (Licensed Professional Counselor) or LCP (Licensed Clinical Psychologist) preferred.

XI.     PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Provides clinical and administrative supervision of REACH Team Leaders and Respite Director.

Coordinates weekly team meetings with REACH teams and respite staff.

Collaborates with regions ID Directors to plan quarterly REACH advisory council meetings. Serves as a liaison to community partners.

Establishes and maintains community linkages to increase crisis response in the community.

Identifies and coordinates training and support needs of the community partners to increase the community’s ability to support individuals with developmental disabilities and co-occurring mental illness and intense behavioral support needs.

Effectively utilizes expertise of psychologists, psychiatrists, team leaders, and other specialists within the community to increase effectiveness of community system.

Ensures collection of data and completion of documentation required by local, state, and federal regulations.

Works collaboratively with Region-V ID Directors, Emergency Services staff, Case Managers and other clinical staff to assure consistent implementation of the REACH model across the region.

Develops, implements, coordinates, monitors and evaluates programs and services.

Develops and maintains policies, procedures and protocols to ensure efficient and effective treatment services that comply with quality assurance, accreditation, licensure standards, and program guidelines.

Ensure programs and services comply with all applicable clinical documentation standards

Supervises development, monitoring, measuring and analysis of outcome indicators for all clinical services.

Supervises, evaluates and assesses the performance of staff.

Assures that staff maintains required agency training as well as developmental training.

Participates in the development of budget and performance contracts for assigned clinical programs.

Coordinates the screening, admission, and discharge of consumers to or from the program.

Maintains liaisons with the agencies, referral sources, contract providers, payers and other health care providers.

Works closely with the regions ID Directors and CSB Management Team to develop and implement effective strategic and marketing initiatives.

Does related work as required.

XII.    PHYSICAL REQUIREMENTS:

Must be able to hear within normal ranges with or without accommodation.  Must be able to talk in order to articulate counseling.  Must be able to see with or without accommodation to observe client. Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. Some walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, handling, sitting, standing, pushing, and pulling.
