CLASS  SUMMARY                                                                                

The Habilitation Specialist provides a variety of functions in support to the ICF/IDD individuals in the areas of psycho-social, functional skills development, community reintegration, monitors functional community living, home-living, vocational training programs, completes habilitation assessments, develops all training objectives, purchase all training materials, develops and monitors all habilitation training binders, facilitate all staff training on habilitation objectives, completes required documentation, and performs administrative duties as assigned by the Residential Services Manager.

DISTINGUSHING ELEMENTS OF THE CLASS:

I.  IMPACT:  Immediate health and welfare of client is directly affected by work of this class. Activities of incumbent that include conducting habilitation assessments and facilitating habilitation objective training, which can promote successful community integration for the client and assist in preventing unnecessary institutionalization.

II.  LATITUDE:  Day-to-day work is performed independently with regular consultation and frequent follow-up from the Residential Services Manager. Work is guided by the Individual Program Plan, program policies, ICF/IDD regulations and guidelines and Program Schedule.

III.  RESPONSIBILITY FOR THE WORK OF OTHERS:  None.

IV.  INTERPERSONAL CONTACTS:  Continuous contact with clients and other staff in planning and providing the clients services. Regular contact with family members and members of other agencies providing services to clients, such as social workers, employers, physicians and service providers.

V.  DIFFICULTY:  Appropriately relating to a variety of client personality types requires use of judgment and discretion in emergency situations resulting from client medical or behavior problems involve timely and responsible decision making. Incumbent must be knowledgeable of active treatment principles, and able to interact with a variety of external human services providers implementing Individual Program Plan objectives and functional skills plan delivery for the client population. The Incumbent develops, in-service and implement habilitation objective training goals for home living, community living, academic and vocational training for all consumers with specified time frame. The incumbent must regularly review and comply with all ICF certification and DSH licensure standards. Assure compliance with local State and Federal law, including OSHA and H-NNCSB Health and Safety regulations and standards.

VI.  COMMUNICATIONS:  Work involves regular oral and written communication with Program Supervisor, Social Worker, clients, vocational service providers, and families. Work also requires incumbent to prepare written documentation such as Progress Notes, Data Collection, Individual Program Plan, business communications, Quarterly progress notes, etc. 

VII.  EQUIPMENT/FACILITIES RESPONSIBILITY:  Incumbent will be responsible for equipment at residential sites such as vehicles, adaptive equipment, TV’s, Microwave ovens, stoves, dishwashers, recreation equipment and associated furnishings.

VIII.  WORKING CONDITIONS:  Working is performed in office, residential facility and possible day treatment setting. Incumbent works at the facility or job site during assigned shift. Frequent outside activities expose incumbent to elements of nature. Some clients can be abusive.

IX.  KNOWLEDGE< SKILL AND ABILITIES (KSA’S):

Good knowledge of intellectual disabilities, and other developmental disabilities.

Thorough knowledge of behavior shaping principles, practices, and techniques.

Good knowledge of the principles of group dynamics.

Ability to communicate effectively and prepare related documentation.

Good ability to assist with the maintenance of client records.

Good knowledge of the theories, practices and techniques related to psycho-social development and rehabilitation of the mentally impaired.

Demonstrated skill in working with adults with disabilities.

Very good oral and written communication skills.

Ability to establish and maintain effective working relationships with clients, families, and other service providers.

Good knowledge of the laws, ICF/IDD guidelines, and regulations.

Ability to train staff various training tools to implement functional skill plans.

Knowledge of Active Treatment principles.

Knowledge of person-centered planning.

X.  LICENSURE:  Valid VA driver’s license. CPR Certification. CPI Training. Medication Training (as required by program). First Aide Certification.

XI.  PRINCIPAL DUTIES AND RESPONSIBILITIES OF CLASS:

Plans and conducts individual development training such as independent living, recreation and leisure activities, pre-vocational training to clients.

Assess client needs, responds to client inquires, and makes referrals to services and resources in the community.

Provides assessment and crisis intervention and referral services.

Advocates for individual and general client rights and services.

Monitors progress and assists with management of client records.

Coordinates appointments in conjunction with the facility nurse.

Interfaces with and supports client family members and/or care givers as designated by the QMRP.

Acts as a back-up for QMRP and completes assigned follows-ups.

Supervises, and assists with administered medications assessments and training.

Completes initial and annual assessments on all clients.

Completes monthly summaries and assists QMRP with completion of quarterly reviews.

Prepares and maintains statistical records and prepares related reports.

Develop, in-service and implement training goals for home living, community living, academic and vocational training for all consumers with specified time frame.

Monitor training of goal plans and program books to assure appropriate procedures, continue functionality and notification of completion of goals. Revise goal plans as indicated by consumer need with QMRP approval.

Provide accurate assessment data for assigned caseload within specified time frame.

Provide accurate time study assessments on assigned consumer caseload by due date and inform consumer of any salary change.

Conduct on-going functional assessments to insure appropriate methodology and implementation.

Attend and request core team meetings to discuss revisions, completion or implementation of training goals.

Assure proper maintenance of Activity training areas. Assure materials are available and appropriate for goal training and integrative activities.

Coordinate transportation of consumers to and from day programs.

Ensure cleanliness/neatness of program areas by scheduling upkeep where necessary throughout the day.

Provide in-service training to ensure consumers/staff are accountable to program schedule and rules, policy and procedures.

Coordinate staffing assignments; make changes as necessary on daily basis according to consumer needs.

Regularly review and comply with all ICF certification and DSH licensure standards. Assure compliance with local State and Federal law, including OSHA and H-NNCSB Health and Safety regulations and standards.

Monitor program building areas for compliance with OSHA standards. Insure quarterly audit is conducted and submitted to Residential Supervisor.

Meets work schedule in a reliable and accountable manner.

Interacts with consumers in a positive and therapeutic manner.

Provide new employee in-servicing on training method as scheduled by Training Services or Residential Services Manager.

Complies with all H-NNCSB policies and procedures.

Does related work as required.

Transports clients as necessary.

XII.  PHYSICAL REQUIREMENTS:  Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a personal computer, calculator, copier, and fax machine. Some lifting from 100-300 lbs, walking up and down stairs, pushing and pulling wheelchairs 275-350 lbs, assisting clients on and off vans, and some kneeling and crawling. May be required to assist with direct client care, as needed, to include: assisting with feeding, toileting and diaper changing, handling non-ambulatory consumers and their assistive technology such as wheelchairs and walkers and managing a wide range of difficult and aggressive behaviors.
