CReST/REACH Crisis Counselor/Coordinator 

Description

The position is responsible for the direct provision of emergency, counseling, and intensive case management/coordination services as part of the Region IV Children’s Response & Stabilization Team (CReST) and the Regional Education Assessment Crisis Habilitation Services. 

ESSENTIAL JOB TASKS

  • Conducts and prepares intake assessments; prepares comprehensive/consultative service evaluations; develops and prepares crisis response plans; completes crisis triage forms.

  • Serves in the role of Certified Pre-Screener, when applicable.

  • Consults with service providers; maintains contact with service providers to monitor behavioral changes and efficacy of crisis plans; connects service providers with additional resources as needed.

  • Identifies and coordinates necessary training and technical assistance needs for providers, natural supports and other components of the system of care.

  • Conducts home visits, school and other community site visits as required and/or permitted.

  • Monitors, documents, and reports on youth and family treatment progress.

  • Prepares, documents in, and fully maintains the electronic health records (EHR) of assigned CReST/REACH cases.

  • Collects and enters required individual consumer data, and submits reports on individual consumer progress, and produces documentation on consumer access and utilization of CReST/REACH.

  • Positively communicates and maintains good working relationships with community partners.

  • Actively participates in all required trainings and meetings with CReST/REACH team members, supervisors, and clinical staff, to include making presentations at clinical education teams.

  • Maintains CReST/REACH caseload as required.

  • Performs work and assists with essential tasks as assigned.

 
Position Requirements

MINIMUM QUALIFICATIONS (EDUCATION, EXPERIENCE, CREDENTIALS)

 

  • Graduation from an accredited college/university with a Master’s Degree in Social Work, Counseling, Psychology, or closely related field;
  • Current Virginia license as an LCSW, LPC, or LCP, or License-Eligible/License-Eligible within 90 days of hire;
  • experience in a behavioral healthcare setting, dually diagnosed children (I/DD and MH) and/or emergency services preferred;
  • Basic knowledge of ES, MH, ID/DD, and SUD treatment services, as well as the CSB/BHA system and structure

SPECIAL REQUIREMENTS

  • clinic and community-based work required.
  • Travel required; must possess valid driver’s license.
  • This position will be based at Crossroads Community Services Board and serving in Charlotte County Courthouse. 
  • This position will cover both the Crossroads and Southside Community Service Boards
  • A program vehicle is provided for travel associated with the position

Starting Salary Range Clinician I $46,160.70 - $50,000.00 - Clinician II $50,891.88 - $55,000.00 VA Licensed LCSW,LPC, LMFT and or LCP
