Description

REACH - Regional, Education, Assessment, Crisis Services, Habilitation

REACH is a 24/7 mobile crisis intervention program that provides crisis support to adults with Intellectual and Developmental Disabilities.  The goal of the program is to prevent unnecessary institutionalization of individuals with Intellectual and Developmental Disabilities and to provide them opportunities to live in the most integrated setting appropriate to their needs and consistent with their informed choice.  REACH provides in-home and community based crisis assistance services to individuals and their families in order to improve the quality of life and decrease facility and hospitalization placements This class provides direct short term crisis residential services for individuals served with intellectual and developmental and behavioral health disabilities. Supervises full and part time staff engaged in residential care.  Responsible for the overall program management of planned and unplanned residential respite component of REACH services.

DISTINGUISHING ELEMENTS OF THE CLASS:

I.  IMPACT: The safety, social functioning and emotional well-being of the clients in residence are directly dependent on the work of this class.  House policies and procedures enforced by the incumbent directly affect the safety of the environment for clients and staff at the residence.  The incumbent is called upon to resolve immediate crisis situations.

II.  LATITUDE: Day-to-day work is performed independently with regular consultation and frequent follow-up from the REACH Director.  Work is guided by comprehensive service plan, program policies, guidelines and instructions.

III.  RESPONSIBILITY FOR THE WORK OF OTHERS: Supervision is exercised over the residential staff which includes full-time and part-time residential staff and may include volunteers and interns.

IV.  INTERPERSONAL CONTACTS: Continuous contact with clients and other staff in planning and providing client services.  Regular contact with REACH clinical team, family members and members of other service agencies providing other services to clients, such as social workers, physicians, and case managers.

V.  DIFFICULTY: Appropriately relating to a variety of client personality types requires use of judgment and discretion.  Emergency situations resulting from client medical or behavior problems involve timely and responsible decision-making.  Incumbent must deal with a variety of other human services officials complicating service plan delivery. Some on call responsibilities.

VI.  COMMUNICATIONS: Work requires incumbent to prepare written documentation such as:  reports, service plans, business communications, client progress notes, etc.  Public relations activities are frequently involved.

VII.  EQUIPMENT/FACILITIES RESPONSIBILITY: Responsible for maintaining and safeguarding residential facility and associated furnishings and supplies.

VIII.  WORKING CONDITIONS: Incumbent performs work at a residential facility.  Must flex their schedule throughout the month to include weekends and evenings.  Facility is isolated, and outside activities expose incumbent to elements of nature.  Frequent calls at home occur. Incumbent is subject to physical and verbal abuse by clients.  Must work an overnight shift at least quarterly.

IX.  KNOWLEDGE, SKILLS AND ABILITIES (KSA'S):

Knowledge of positive behavioral supports.

Knowledge of ID and MH systems.

Knowledge of Person Centered practices.                            

Knowledge of Medicaid regulations, and DBHDS Licensure standards.

Thorough knowledge of the principles and practices related to the operation of a residential facility for adults with intellectual disabilities.

Thorough knowledge of community, agency, regional and state resources and programs for the resident population.

Good knowledge of client management guidelines for Virginia.

Good knowledge of office management procedures.

Good knowledge of intellectual disabilities, autism, and other disabilities.

Good knowledge of time management techniques.

Demonstrated skill in working with disabled adults.

Very good oral and written communication skills.

Ability to establish and maintain effective relationships with clients and staff.

Ability to function calmly in stressful situations.

Ability to manage program operating budget.

X.  LICENSURE: Valid CPR and First Aid certification.  Valid Virginia driver's license.

XI.  PRINCIPLE DUTIES AND RESPONSIBILITIES OF CLASS (ILLUSTRATIVE ONLY):

Responsible for the implementation of REACH Respite Home and other respite supports for individuals with intellectual and developmental disabilities and intense behavioral needs.

Provides clinical and administrative supervision of REACH respite staff.

Coordinates with REACH Teams on admission into residential respite facility.

Plans and implements structured day programming for all participants.

Coordinates transition planning for individuals utilizing REACH residential respite services.

Maintains contact with families and/or providers upon admission to respite services.

In collaboration with REACH teams, identifies on-going support and training needs of families and providers who utilize REACH respite.

Maintains linkages and relationships with community partners.

Attends all required meetings with Team Members, REACH Director, and clinical staff.

Identifies and coordinates necessary training for respite staff.

Ensures the collection of required data and documentation on consumer access and utilization of REACH services.

Documents supervision of all staff.

Participates in securing and maintaining orderliness of facility.

Coordinates shopping for food/facility needs by staff and/or clients.

Insures client medical needs are met and medical information is documented in client record.

Coordinates positive client relationships within community groups, civic leagues and neighborhood businesses.

Performs emergency first aid procedures when necessary.

Takes inventory of facility supplies and equipment as needed.

Coordinates maintenance of facility vehicle.

Reports all house maintenance or repair needs to Facilities/Maintenance Manager.

Monitors program operating expenses.

Does related work as required.

Transports clients as necessary.

XII.     PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. Some running after clients, walking up and down stairs, pushing and pulling wheelchairs, assisting clients on and off vans, and some kneeling, crawling, bending and stooping, lifting heavy boxes and containers that hold large quantities of foods, supplies of more than 50 pounds. It may be necessary to help lift clients who are in braces/ or in bed due to surgery or injury.
