JOB SUMMARY: This position is responsible for the coordination and delivery of clinical behavioral health services designed to meet the wellness and recovery needs of the consumer, including services provided to victims of violence. Depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumers family members.
ESSENTIAL FUNCTIONS
Intake:
Preparation of intake evaluations and diagnostic assessments
Clinical Services:
Provide individual, group and family therapy; psycho-educational groups.
Consult with psychiatrist regarding program clients as needed
Provide therapy at satellite offices, if needed.
Documentation:
Clinical documentation should be maintained in Anasazi, the agencys electronic health record software, and should include: the assessment, diagnosis form, treatment plan, progress notes, collateral contacts, and termination summary.
Case Management:
Maintenance of continuous communication with clients, including outreach for hard to engage clients
Provide family support and advocacy at Grand Jury proceedings.
Act as liaison to other agencies as it pertains to referrals.
Provide information and referral to appropriate community resources
Productivity:
Participate in weekly individual and peer supervision
Clinicians are expected to spend 65% of their time in face to face consumer contact and to provide routine productivity reports to their supervisor.
All schedules, statistical reports/logs, and clinical reports are to be submitted within proscribed timelines.
Documentation should be completed in Anasazi within 24 hours of service delivery.
Supervision:
Possible supervision of graduate students fulfilling field work requirement toward Masters Degree in Social Work
OTHER DUTIES
Represent agency at Multidisciplinary Team meetings
Participate in committees through the Prosecutors Office/Child Advocacy Center as appropriate
Participate in Quality Improvement process for this program
Other related duties as assigned
