Description

POSITION SUMMARY: 
This position has responsibility for the management of patient care provided by department personnel. RN/ICU assesses, plans, implements and evaluates the nursing care of the departmental patients in order to ensure that patients and families receive an optimum level of care. RN/ICU must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the department's policies and procedures. RN/ICU practices relationship centered care in concert with the University General Hospital core values of compassion, integrity and innovation and demonstrates excellent customer service standards.

PATIENT AGE GROUP SERVED:
The ICU RN must be able to provide care to patients of all ages according to the patient population served (i.e. Infant (29 days – 12 months), Pediatric (13 months – 12 years), Adolescent (12-18), Young Adult (19-40), Middle Adult (40-65), Mature Adult (65 years and older).

DUTIES AND RESPONSIBILITIES:

  • Applies the nursing process to a group of patients.  Evaluates outcomes of patient care, consults with other disciplines as required and adjusts the plan of care as necessary to ensure optimal service
    • Effectively identifies predictable problems and plans assigned tasks as needed
    • Performs nursing assessments and initiates the plan of care
    • Recognizes changes in patient condition and initiates activities to facilitate appropriate responses
    • Communicates the patients condition and/or change to appropriate staff and/or physician, as needed.
  • Ensure appropriate delivery of patient care for a group of patients according to policy and procedure; assigns and/or delegates patient care assignments to licensed and non-licensed nursing staff, based on acuity, needs of the patient, staff availability and qualifications.
    • Makes patient care and related work assignments to the appropriate department staff; provides direction, and reviews all work, as necessary.
    • Responds promptly to changes in workload, adjusting assignments as necessary to achieve optimal productivity and efficiency.
    • Prioritizes and plans all work steps to ensure success; considers time, supplies and support required from other individuals and departments in setting up work plan and completing duties.
    • Provides pertinent shift report to other members of the team.
    • Assures appropriate follow-up of care for patients assigned to LVN’s and for tasks delegated to non-licensed care givers.
  • Participates and facilitates in the case management process and cost containment for patients.
    • Makes appropriate referrals upon admission and throughout hospitalization
    • Considers all available options and makes appropriate decisions
    • Assist in the containment of hospital costs through appropriate use of supplies, recovery of patient charges, and conservation of supplies.
    • Assist in the evaluation of patients for appropriate level of care and communicates with others on the health care team.
    • Communicates pertinent information regarding patient’s progress and discharge planning needs to appropriate hospital and medical staff.
    • Evaluates patient progress in light of the plan of care and coordinates care to achieve desired outcomes.
  • Cooperates with other personnel to achieve department objective and maintain good employee relations with collaborative integration of the health care team.
    • Maintains harmonious relationship with staff members, providing support and assistance to ensure quality customer service.
    • Demonstrates attitude of cooperation and professionalism.
    • Demonstrates flexibility in working hours and days scheduled in order to meet needs of the department.
    • Seeks and accepts input from Director/Supervisors regarding interpersonal relationships that affect the department’s harmony and serviceability.
  • Demonstrates accountability for direct patient care and evaluation of patient outcomes.
    • Follows established procedures for all treatments, performance of procedures, and medication administration.
    • Assures the patient treatment plan is followed.
    • Implements prescribed treatments accurately and in a timely manner.
    • Evaluates patient response/outcomes to procedures performed, treatments, medication administration and other aspects of the treatment plan.
    • Recognizes changes in patient condition and initiates activities to facilitate appropriate response.
  • Documents all aspects of patient care efficiently and effectively
    • Ensures documentation and charging of resource/supplies.
    • Documents complete patient assessment upon admission within the time frame as per policy.
    • Documents all medications, treatments, and evaluation of such.
    • Determines and documents patient problem list, interventions and evaluation.
  • Demonstrates professional practice and conduct at all times.
    • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of the latest trends in the field of expertise.
    • Takes responsibility to regularly update self on knowledge of department and hospital communication changes
    • Reports to work on time and absences are not to exceed hospital standards.
    • Conducts job duties in accordance with established hospital, department, and unit policy and procedures.
    • Demonstrate leadership as a team leader.
    • Serves as a resource to coworkers.
    • Participates in unit development and problem solving.
    • Communicates effectively with members of the healthcare team.
    • Complies with legal and professional regulations.
    • Demonstrates behaviors that promote teamwork within department and interdepartmentally, in order to best serve the patient.
    • Attends and participates in required number of staff meetings per year.
    • Performs other duties as assigned.
  • Participates in Performance improvement as appropriate (i.e. data collection, quality control, etc.)
    • Is able to articulate departmental PI/QC activities.
    • Is able to articulate one of the top 4 hospital-wide PI activities.
  • Demonstrates Age Specific Competency for the Geriatric.
    • Note sensory  motor function changes and facilitate care according
    • Assures dentures are in during use of ambu bag.
    • Utilizes fall precautions.
    • Monitor for skin breakdown and provide care as needed.
    • Assess for an adequate swallow.
    • Assess physical and psychosocial changes associated with aging and make recommendations to other interdisciplinary care givers.
    • Involve the patient in planning and providing self care when possible and include primary at-home caregiver.
    • Knowledge of growth and development
  • Demonstrates critical care skills for managing acute ill patients.
    • Demonstrates proficiency in identification and management of patients with dyrhythmias.
    • Safely manages patients with mechanical ventilation.
    • Demonstrates working knowledge of vasoactive and anti-arrhythmic medications and titration procedure.
    • Proficient in performing emergency protocols for RN’s (ACLS)
    • Demonstrates knowledge of specialized procedures performed in critical care area.
    • Is proficient in managing hemodynamically unstable patients and demonstrates technical and working knowledge of pulmonary artery catheters, arterial lines, and measurement of cardiac output.
  • Demonstrate the components of the hospital’s core values of compassion, integrity and innovation.
  • Demonstrates customer service standards by smiling, making eye contact, greeting each patient and visitor, seeing out patient and visitor contact, displaying appropriate body language at all times, taking time to go above and beyond for one patient/visitor each day, honoring diversity, and thanking each customer.
  • Follows all safety rules while on the job.  Reports all accidents and corrects minor safety hazards.  Communicates with peers and management regarding any hazards identified in the workplace.
  • Performs other duties as assigned.
 
Position Requirements

EDUCATION REQUIREMENTS:

RN Licensure in the State of Texas, with a BSN preferred.

EXPERIENCE REQUIREMENTS:

Six months to one year clinical experience in an acute care setting required.

CERTIFICATES, LICENSES, SKILLS AND ABILITIES REQUIRED:

  • A current license to practice as an RN in the State of Texas
  • Current BCLS
  • Current ACLS

WORKING CONDITIONS:

ICU RN must be able to work in a stressful environment.  They may be exposed to biohazards and chemicals as outlined in the Infection Control, Safety, and MSDS Manuals.  Standard Precautions is required, with significant potential for exposure to latex.  This position may require duties including lifting and carrying up to 35 pounds, pushing, and pulling more than 100 pounds.  It may also require frequent standing, walking, balancing, stooping, bending, kneeling, and reaching.  Good visual and auditory acuity as well as good manual dexterity and the ability to be readily understood are essential.
