Nurse Liaison
Right at Home in Home Care and Assistance Company has an opening for an outgoing and energetic Nurse Liaison to join our team. In this exciting role you will use your effective communication skills both in our office and in the field. As a front-line representative of the agency, you will support the agency’s positive image in the community.
Duties will include:
Identifying training gaps and supervising and training caregivers in the field
Working autonomously to set your own schedule of visits
Documenting visits in a timely and accurate manner
Communicating with all stakeholders to ensure smooth delivery of care and customer satisfaction
Proactively reaching out to referral sources and participating in community events
Requirements to be considered for this position:
LPN, but RN is preferred
Two years experience but will train the right candidate if a new grad
Desire to work with seniors
Experience working with seniors a plus
Supervisory experience preferred
Community nursing experience preferred
Excellent oral and written communication skills and a strong ability to work with diverse personalities
Personal reliable and insured transportation and a valid driver’s license
Computer literacy
Willingness to travel 90%
