Description

Case Management services provide communication, logistic, and advocacy support to patients working with interdisciplinary team members to improve health outcomes. Case Managers partner with patients to navigate resources, reduce barriers to care and improve health outcomes. The Case Management Services Coordinator will uphold the mission and values of Case Management in collaboration with the Director of Case Management and the Case Management team. Through Coaching, Leading, Mentoring, and Collaborating with the Case Management team, the Case Management Services Coordinator develops, trains, and upholds the standards of case management care coordination.

Case managers use a holistic, patient centered approach to identify barriers to health outcomes. Using evidence-based and harm reduction rooted practices, Case Managers provide care coordination in partnership with patients to meet their individual goals. Case Managers work with patients through completing assessments, care planning, service enrollment, and continued case review with the goal of improving patient self-sustainability and improved health. Case managers work with interdisciplinary teams and community resources to address identified barriers and vulnerabilities in support of the patents identified goals through care planning and continued support as needed.

The Case management Services Coordinator upholds all case management standards and expectations through providing case consultation, reviewing/auditing documentation and observing case management practices. The Case Management Service Coordinator will serve as a professional resource and system of support for case managers. This includes taking on cases as needed or assigned to ensure work flow and case management services are continually provided and sustained.   The Case Management Services Coordinator will maintain competency in the following areas:

Position Requirements

* Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES’ policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.

  • 1+ year of professional experience in the case management or treatment of individuals in any of the following care coordination areas: Medical/Nonmedical, HIV+, Housing, Behavioral health, or targeted care coordination for vulnerable populations such as pediatric, geriatric, or LGBTQA+ required.
  • 1-3 years’ experience supervising teams and implementing case consultation preferred.
  • Computer literacy in Word, Excel, and Medical record keeping required
  • Ability to complete CHW 1 Certification within 6 months of hire required.
  • Bachelor's or Master’s degree in human services or medical field, LSW, MSW, CHW, licensure/certification preferred but not required.
  • Proficient in Spanish/English, written and verbal preferred but not required.
 
About the Organization Northern Nevada HOPES is a nonprofit community health center operating two clinics and a transitional housing community in the heart of Reno, Nevada. Our one-stop-shop integrated care model helps us meet our patients where they're at and make medical and behavioral health care more accessible to medically under-resourced populations.

Services we offer include adult and pediatric primary care, chronic disease management, nutrition services, behavioral health counseling, substance use counseling and treatment, case management, on-site pharmacies, and more! We accept most insurance plans, including Medicare and Medicaid, and offer discounted services for uninsured patients, so anyone who needs care can access our services.

Salary Range $52,000 - $66,000 per annum
