Guardianship Volunteer Advocate 

About the Organization Thrive Alliance helps individuals access resources and support services that improve their lives and builds stronger communities. As the "umbrella" over several programs, we provides services that help the elderly, disabled, and low-income individuals in 5 counties of south central Indiana: Bartholomew, Brown, Decatur, Jackson, and Jennings. These programs include:

Aging and In-Home Services - as the local Area Agency on Aging, Area 11, Thrive Alliance provides Care management and resources to assist those over the age of 60 to remain at home or in a community setting.

Aging and Disability Resource Center (ADRC): a highly visible and trusted place where people can turn for information on a full range of support options.

Adult Guardian Services: Provides guardianship services for disabled adults in need of support and legal assistance.
Nutrition Services: Provides congregate meal site where seniors can come have a hot meal and interact with others in a comfortable environment. Educational program often offered. Current meals sites include Nashville, IN, Seymour, IN, Crothersville, IN, North Vernon, IN and Columbus, IN.
First Steps: Provides case management, assessment, advocacy, and resources to assist families with a child under age 3 diagnosed with a developmental disability
Caregiver Support: Provides support and education to families and caregivers of individuals dealing with dementia
Housing Partnerships: Provides safe and affrodable housing to low-income, seniors and disabled individuals

Description

DESCRIPTION OF ASSIGNMENT: When Thrive Alliance, becomes the legal guardian, and therefore all responsibilities, for specific individuals who reside in Bartholomew County. These individuals are unable, for various reasons, to make appropriate decisions for themselves, and no longer have families or others who are willing or able to assist them. The GVA monitors the affairs of, and assists with decision making for, his/her assigned client as needed; and works toward the goal of having his/her client's needs met while also resolving any unmet issues.

SPECIFIC DUTIES:

  • Visit the client at least 1 time a month and attempt to establish a meaningful relationship.

  • Attending quarterly client care conferences and assisting in the development and periodic review of client's service plan.

  • Evaluate the adequacy and appropriateness of living arrangements and care for client and take any necessary measures to improve the arrangements when found inadequate; work with Guardianship staff to accomplish a change of residence if necessary.

  • Accompany client to medical or other appointments when appropriate and needed.

  • Promptly notify Adult Guardianship Services Staff of significant changes in client's living and/or medical conditions.

  • Assist in purchasing personal items from client's personal funds.

NOT ALLOWABLE:

  • Any type of personal or hands on care, such as bathing, dressing, grooming, feeding, medication dispensing, etc.

  • TIME COMMITMENT: On average 2-4 hours per month for each client. At least 1 visit per month for a minimum of one year.

    TRAINING: Training will consist of 15-24 hours. The initial session covers the National Guardianship Standards and Guardianship responsibilities, which is a guide to the role of the GVA. The GVA will submit monthly reports as a part of the training process. The GVA will attend periodic training sessions and support meetings of the program.

    REPORTS REQUIRED: A Monthly Report indicating each visit is required. It can be submitted via email or entered on the website where notes are recorded. Regardless of which method is chosen, the monthly report of hours must be submitted by the end of each month.

 
Position Requirements

QUALIFICATIONS & SKILLS:

  • Minimum of 21 years of age

  • Consent to criminal background check

  • Proof of valid driver's license and auto insurance

  • Empathy; genuine desire to work with, and ability to communicate with, the elderly and disabled population

  • Ability to respect client confidentiality

  • Good listening and communication skills
