Share Email Opening Title CNA-2:30 pm-10:30 pm-Assisted Living Facility  Full-Time/Part-Time Part-Time   Shift Evenings   Description The Home Health Aide ensures quality and safe delivery of assisted living health care services (located in Buffalo, Illinois). Participates in development and implementation of client plans of care per health care agency policy and procedure. Participates in client case conferences according to agency policy and procedure, as appropriate. The provided home health aide services reflect client plans of care. Information regarding client plans of care is submitted to the Registered Nurse in a timely manner. Client clinical record documentation reflects delivery of quality and safe health services. Implements current home health aide services. Client plans of care are discussed with the Registered Nurse on a regular basis. Client clinical records are documented per home health care policy and procedure. Client assignments and reports are received from the Registered Nurse.   Position Requirements High school diploma/GED required. Current Illinois CNA certification required. Experience in home health care or related health care field preferred. Valid Illinois driver's license.
