Information regarding client plans of care is given to home health care staff members, clients, and client families in a timely manner. Clients and client families are involved in developing client plans of care whenever         possible.  Implements current occupational therapy practice. Assist the physician in evaluation of the patient’s level of function by applying diagnostic and prognostic procedures. Guide the patient in the use of therapeutic, creative, and self-care activities for the        purpose of improving function. Observe, record and report to the physician the patient’s reaction to treatment and any  changes in the patient’s condition. Instruct other health team personnel including, when appropriate, home health aides    and family members in appropriate, home health aides and family members in certain  phases of occupational therapy in which they may work with the patient.     Position Requirements Bachelor of Science Degree and graduate from an accredited school of occupational therapy required. Current State of Illinois Occupational Therapist license required. Registration with the American Occupational Therapy Association required. Minimum of one year previous experience required. Evidence of independent nursing practice in delivering nursing care required. Current Illinois Driver's License. Current automobile liability insurance. Visual/hearing ability sufficient to comprehend written/verbal communication.  Ability to perform tasks involving physical activity, which may include heavy lifting and    extensive bending and standing.  Ability to deal effectively with stress.   Location Jacksonville, IL   About the Organization Alterna-Care Home Health System is the most comprehensive home health agency in central Illinois.
