Description

The Home Health Registered Nurse is responsible for the overall case management and collaboration of the care of their home bound patients.  General position responsibilities also include:

Ensure quality and safe delivery of home health care services.

  • Develop, implement, and evaluate client plans of care per agency policies and procedures.
  • Complete nursing assessment forms in a timely manner per agency policies and procedures.
  • Develop individual nursing plans of care prior to client case conferences.
  • Conduct client case conferences in a timely manner according to agency policies and procedures.
  • Ensure individual nursing plans of care reflect client needs.
  • Promptly share information regarding client plans of care with employees, clients, and families.
  • Clinical record documentation reflects delivery of quality and safe home health care services.
  • Conduct registered nurse supervisory visits of home health aides per agency policies and procedures.

Implement current nursing practice.

  • Assess client conditions during every home health care visit.
  • Communicate client assessments to appropriate individuals.
  • Implement nursing interventions to meet client needs and changing conditions.
  • Complete client clinical record documentation per agency policies and procedures.
  • Accurately transcribe, implement, and evaluate physician orders in a timely manner.
  • Complete referrals of clients to other health care providers per agency policies and procedures.
  • Document referral information accurately, completely, and promptly.
  • Implement infection control and safety measures per agency policies and procedures.
  • Promptly communicate recommendations and requirements for additional home health care services for clients to the appropriate individuals.
  • Implement clinical care per agency policies and procedures.
  • Monitor client care for compliance with defined client plans of care.
 
Position Requirements
  • Graduate of an accredited school of nursing required.
  • Current license to practice as a Registered Nurse in the State of Illinois required.
  • At least one (1) year of experience as a registered nurse, preferably in an acute care setting.
  • At least two (2) years of experience in a home health setting preferred.
  • Valid Illinois Driver’s License
  • Current automobile liability insurance
