SUMMARY: Under the supervision of the Office Manager the Treatment Coordinator will identify and assess clinic patients who have been recommended for urgent care according to CAMBRA guidelines and the tracking of such cases to ensure that treatment plans are finished in a timely manner. The Treatment Coordinator is also responsible for tracking all cases referred out for hospital dentistry by their primary care provider. The Treatment Coordinator will be responsible for follow up on all outside referrals for active patients of record. And other duties deemed necessary by Vice President of Dental Services and the Dental leadership team.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

· Works with patients and their families to utilize Dental health care resources, social services and com-munity resources appropriately to optimize and improve on their health status.

· Responsible for developing a resource and referral system within the dental clinics, as well as estab-lishing on-going relationships with the patients and caregivers and other community sources of sup-port.

· Responsible for establishing an effective recall system for patients at all dental sites.

· Plans and develops a tracking and reporting system for the case management of all dental programs such as PDI, Angel Fund, CAMBRA, Well Child, Fast Track, Title Three, etc.

· Identifies patients who are urgent care with multiple diagnoses and health issues and helps expedite care.

· Assesses the social needs of each patient (family) and his or her coping and adaptive abilities, formal and informal support-systems, and self care abilities.

· Determines the barriers each patient faces (literacy, language, income, etc) and helps the patient de-velop a plan to overcome them.

· Establishes baseline data on the health and utilization patterns of these patients.

· Provides early intervention for patients such as Well Child, Recalls, and Education etc.

· Facilitates and promotes effective communication between the patient and the dental provider and collaborates with the provider in the development of an effective patient education program.

· Meets regularly with patients and assists them in fulfilling recommendations for their dental treat-ment plan. Including, but not limited to: explanation of special instructions, assistance with transpor-tation needs, translation.

· Acts as liaison and facilitator between patient and interdisciplinary care team.

· Provides information to other agencies as appropriate.

· Communicates regularly with Provider, Office Manager, and Administrative Site Director.

· Facilitates access to financial coverage for needed care: Coordinates and participates in Quality As-surance activities.

· Attends and coordinates chart review and case management with provider and other involved staff members.

· Other duties and responsibilities as designated by supervisor.

· Ability to accurately chart patient information.

· Ability to promote effective communication between the patient and the primary care provider.

· Ability to effectively represent ODCHC’s interests in the community and maintain effective working relationships among coworkers, public, private, and professional groups.

· Strong computer skills with specific aptitude in Microsoft Office Suite and/or Electronic Medical Rec-ords (EMR).

· Adheres to ODCHC’s policies and procedures.

 

EDUCATION AND EXPERIENCE:

· ODCHC Dental Experience preferred.

 

SUPERVISORY RESPONSIBILITIES: None.

 

SUPERVISION AND SUPPORT: The Treatment Coordinator reports directly to the Office Manager and is an integral member of the Dental Care Team.

 

PHYSICAL REQUIREMENTS: This is largely an office-based position. The physical requirements described are representative of those needed to successfully perform the essential duties of the position. Reasona-ble accommodation will be made to allow otherwise qualified candidates to perform these functions.

 

· Ability to communicate via telephone, video, and/or in-person.

· Vision adequate to read documents, computer screens, forms.

· Ability to remain stationary for extended periods.

· Ability to lift, carry, or otherwise move up to 25 pounds.

· Ability to use keyboard and view computer screens for extended periods.

· Ability to travel locally and long-distance as needed.

· Ability to move around offices and clinics as needed.

 

The hourly pay for this position ranges from $22.00 to $29.10. All new hires will begin at $22.00 per hour.
