ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Evaluates and documents patient’s health care needs and recommended interventions based on telephonic assessment of patient reported signs and symptoms.
- Plans appropriate interventions and determines most appropriate level of care based on assessment; collaborating with other disciplines when needed.
- Coordinates services with other departments for follow-up based on patient’s needs; including scheduling appointments and directing patient to emergency services.
- Organizes coverage requests from all clinics and tracks in Outlook Calendar.
- Keeps track of Triage RNs schedules and assignments and tracks in Outlook Calendar.
- Facilitates regular meetings of Triage RNs and provides ODCHC updates.
- Maintains accurate records of “Remote Triage Checklists” from all clinics and store on S: Drive.
- Corresponds with Triage RNs and RN Clinic Managers by emailing weekly assignments.
- Maintains daily communication with Triage RNs and available as a resource.
- Supports providers and patients within ODCHC by being part of a multidisciplinary team.
- Serves as a clinical liaison for patients, families, primary care providers and other members of the patient’s care team.
- Serves as a clinical resource for non-clinical call center staff.
- Communicates regularly with care teams at ODCHC locations, supervisor and other Triage RNs.
- Assists with ODCHC Clinical RN inbox duties, including but not limited to: medication refills per protocol, results, and patient advice requests as assigned by supervisor.
- Adheres to ODCHC Triage policy when working with established and non-established patients.
- Adheres to California Board of Registered Nursing regulations when providing telephone triage to patients out of state.
- Participates in Quality Improvement activities as assigned by supervisor.
- Adheres to ODCHC’s attendance, remote worker, and employee health policies.
- Communicates with supervisor and other Remote Triage RNs when changing work location, work hours, or contact information.
- Other duties and responsibilities as designated by supervisor.
QUALIFICATIONS AND EXPECTATIONS:

- Strong computer skills with specific aptitude in Microsoft Office Suite and/or Electronic Medical Records (EMR).
- Adheres to ODCHC’s policies and procedures.
EDUCATION AND EXPERIENCE:
- Degree in nursing and licensed by California Board of Registered Nursing.
- Demonstrates clinical competence in essential duties.
- At least one year of experience in triage within ODCHC.
- Bilingual in Spanish preferred.
- Complete Telephone Triage and other trainings as assigned.
CERTIFICATES, LICENSES, REGISTRATIONS:
- Current Registered Nurse license from the State of California
- Current CPR certification.
SUPERVISORY RESPONSIBILITIES: Co-supervision of all RN – Triage with the Director of Clinical Services.
SUPERVISION AND SUPPORT: The RN Triage Lead reports directly to the Director of Clinical Services and is an integral member of the Clinical Care teams and Clinical Services Department.
PHYSICAL REQUIREMENTS: This is largely an office-based position. This position can be done remotely in an approved remote location. The physical requirements described are representative of those needed to successfully perform the essential duties of the position.
