SUMMARY:

Licensed Vocational Nurses work directly with providers and and Open Door's unique Care Teams. These individuals work closely with patients and providers to ensure the best comprehensive care is received. The LVN functions throughout the day between many diverse areas of responsibility including but not limited to: patient observation, preparation/assistance, and patient education. LVNs work one-on-one with patients during observation and work to record and update patients vitals and medical history. Within this observation time they assess and maintain patients needs and comfort levels. The LVN prepares the room and equipment for each new patient and works alongside the Providers during examinations. LVNs administer immunizations and medications as trained to do so. Open Door works hard to educate staff, patients and the community in prevention and treatment. Through patient interaction and consultation, LVNs play an integral part in the front line of this education. Basic computer skills needed. LVN license and current CPR certification required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Prepare patients for examinations.

  • Note brief history of the reason for the visit in the patient’s chart.

  • Take and record vital signs, height and weight, hearing test, EKG, etc., and notify provider of abnormal findings.

  • Assist clinical staff with examinations and procedures.

  • Take history for prenatal and family planning patients.

  • Administer medications per provider order, with exception of IV medications.

  • Update patient charts in regards to allergies, medication use, and immunization history at each visit.

  • Administer immunizations understanding orders from provider.

  • Educate and counsel patients concerning their disease, treatment, and prevention of disease.

  • Clean and sterilize medical equipment and instruments.

  • Utilize tracking system for pap smears, immunization and breast abnormalities.

  • Patient screening for triage.

  • Follow-up on abnormal findings with provider.

  • Stock each exam room with supplies.

  • Participate in meetings and program development, case management, such as immunizations, family planning, CHDP, hypertension and diabetes.

  • Answer patient’s questions after consultation with the provider, or as per established protocols.

  • Assist in health education classes as necessary.

  • Initiate routine lab tests after triage of a patient, such HCG, Micral, Urine Dip, and Rapid Strep etc.

  • Participate in Quality Improvement as required.

  • Adhere to ODCHC attendance policy

  • Other duties and responsibilities as designated by provider/supervisor.

     

    QUALIFICATIONS:

  • Ability to work as a member of a team in order to communicate and/or solicit pertinent information to and from other team members and support team decisions.

  • Ability to communicate effectively and exercise sound and responsible judgment.

  • Excellent interpersonal skills, written and verbal.Ability to establish constructive working relationships with all levels of management and employees in a staff of varied and diverse backgrounds.

  • Ability to handle difficult or confrontational situations in a calm, consistent, and equitable manner.

  • Ability to write routine reports and correspondence.

  • Ability to accurately chart patient information.

  • Ability to promote effective communication between the patient and the primary care provider.

     

    EDUCATION and/or EXPERIENCE:

  • High school diploma or GED.

  • Completion of an LVN training program leading to licensing asLicensed Vocational Nurse.

     

     

     

    CERTIFICATES, LICENSES, REGISTRATIONS:

  • Licensed Vocational Nurse license, venipuncture certificate, , IV therapy certificate and injection certificate.

     

     

    SUPERVISORY RESPONSIBILITIES:

    NONE

     

    PHYSICAL DEMANDS:

  • Speaking and hearing sufficient to communicate effectively by phone or in person, at normal volumes.

  • Vision adequate to read correspondence, computer screen, forms etc.

  • Good manual dexterity.

  • Ability to sit or stand for extended periods of time.

Wage Range Hiring Range $24.51 to $29.40
