Description

Patient Care Coordinators are a vital part of our health care teams. They serve as a resource person for providers and staff, providing hands-on coverage as well as support and guidance to get things done efficiently They are responsible for the flow of patients through the various services provided by the team, from medical exams to refill requests to connection with case managers, with responsibility for panel management, recalls, and arranging routine care appointments. The coordinator assists providers and staff to make sure they have the resources and information they need to provide the highest quality care. Working closely with the team RN and support staff, the coordinator provides after-visit education, assistance with referrals, lab tests and imaging orders, and assists with reviewing/managing provider MyChart messages and In baskets. While supporting patients, the coordinator’s primary responsibility is to make sure the team is cohesive as a team and functioning at top efficiency and effectiveness, improving the care experience for providers, staff and patients alike. This position is open to current ODCHC Medical Assistants with at least two years of experience. Wage dependent on experience.

 

Position Requirements

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

  • Expertly interviews patients and document in the EMR which includes the following: measures vital signs such as pulse rate, temperature, blood pressure, respirations, weight, and height, and LMP for females using the age appropriate tools (auscultator, B/P) to ensure accurate results and record information on patients' chart.

  • Interview the patient and ask appropriate questions based on chief complaint/diagnosis and anticipate provider’s needs regarding patient care and respond accordingly.

  • Inquire about signs and symptoms leading to the performance of the orthostatic vital signs; perform expertly and document appropriately in EMR.

  • Perform the following: autoclaving, spore check, cleaning the autoclave, and the proper wrapping/packaging of instruments.

  • Prepare exam rooms between patient visits and stock patient exam rooms consistently.

  • Operate electrocardiograph (EKG), oxygen saturation monitors, vision and hearing tests, ear and eye irrigations, peak flow, nebulizer, and sphygmomanometer with expertise.

  • Administer all medications/injections and immunizations per ODCHC policy and as directed by practitioner/registered nurse, using the proper calculations of doses and the seven rights of administration of all medications prescribed.See “Qualifications” below.

  • Set-up and assist on all minor procedures (see MA I) and surgical procedures including circumcisions, incision and drainage (I & D), toe nail removals, endometrial biopsy’s, colposcopy, and cleaning per clinic site.

  • Expertly perform basic dressing changes, as well as ear lavage per provider order and under the direct supervision of a provider.

  • Expertly set up and assist in age appropriate well child exams/CHDP including immunizations and accurate documentation of information

  • Follows guidelines for universal precautions including hand washing and use of Personal Protective Equipment (PPE) as appropriate.

  • Perform HCG, Urine Micral and Rapid Strep and QC according to site specific lab guidelines and expertly obtain specimens and cultures for the lab.

  • Expertly obtains diabetic labs at the beginning of patient’s visit (i.e. Dermal stick, HGB A1c, blood sugar, urine micral) per DM HCM protocol.

  • Schedule appointments, recalls and follow up visits as per clinic procedure, referencing panel reports as directed.

  • Keep medical areas clean and properly disinfected following standard practices of medical asepsis, including exam rooms, hallways, and other areas.

  • Perform daily chart scrubbing and updating of the Health Care Maintenance function (HCM) in EMR and obtain hospital/ED and imaging reports as appropriate.

  • Expertly and consistently perform EPIC workflows for MA duties including standard intake, telephone call documentation, prepping/routing refill encounters, PHQ, MAR, Meaningful Use and managing staff messages from the provider in basket.

  • Initiate and follow to completion; Prior Authorizations and TARs as needed for medications and procedures per ODCHC guidelines.

  • Adhere to ODCHC attendance policy

  • Maintain expert understanding of and adherence to HIPAA, HITECH and CA regulations on privacy.

  • Adhere to the MA Scope of Practice in California.

  • Other duties and responsibilities as designated by supervisor.

     

     

    QUALIFICATIONS:

     

  • Ability and desire to work as a member of a team in order to communicate and/or solicit pertinent information to and from other team members and support team decisions.

  • Strong knowledge of the MA Scope of practice in California.

  • Effective communication skills, written and verbal.

  • Proficient computer skills; internet, EPIC, Word and Outlook.

  • Exercise sound and responsible judgment.

  • Excellent interpersonal skills.Ability to establish constructive working relationships with all levels of management and employees in a staff of varied and diverse backgrounds.

  • Ability to handle difficult or confrontational situations in a calm, consistent, and equitable manner.

  • Ability to consistently and accurately chart patient information.

  • Knowledge of OD referral process and follow up.

  • Ability to promote effective communication between the patient and the primary care provider as well as other clinic staff.Demonstrates good customer service skills.

  • Knowledge and ability to help patients understand discharge instructions, patient informational handouts and to explain and clarify preparations for upcoming procedures ordered by the provider.

  • Ability to write routine reports and correspondence

  • Injection Certification obtained by six months of employment. Knowledgeable of all injection locations (i.e. SQ, IM, ID), the seven rights of administration “The right medication, the right dosage, the right route, the right site, the right patient, right documentation and right to refuse”.

     

     

    EDUCATION and EXPERIENCE:

     

  • High School diploma or GED

  • Completion of accredited Medical Assistant training program, State Certification, or has successfully challenged the state board examination or passed OD skill assessment for MA III (required).

  • Successfully completed or challenged all ODCHC MA Clinical Modules with score of 85% or higher.

  • Three or more years of experience providing direct patient care for Open Door Community Health Centers.

  • Injection certificate (required)

  • Current CPR Certification (required)

  • Medical Assistant Certification or Certificate of Completion of Accredited Medical Assistant Program(optional)

  • Venipuncture/dermal stick certificate (optional)

     

     

    PHYSICAL REQUIREMENTS:

     

  • Specific vision abilities required by this job include close vision, color vision, and the ability to adjust focus. Vision adequate to read medication labels, electronic and written materials, medical equipment readings & results.
  • The employee is frequently required to communicate (talk and hear) in person and by phone, at normal volumes.
  • Activities include sitting, standing and walking. These activities, to various degrees, extend through all work activities. Standing and walking are performed frequently throughout the work shift.
  • Activities require the employee to continuously and repetitively sit for long periods of time.
  • Activities frequently include lifting and/or moving up to 10 pounds and occasionally lift and/or move up to 40 pounds.
  • Activities frequently require the employee to use hands & fingers, handle or feel objects or tools, manipulate & operate standard medical equipment including, but not limited to, exam tables, sphygmomanometers, syringes and stadiometers. The employee frequently is required to reach with hands and arms. Kneeling, crouching, stooping and crawling are required on an occasional basis.
  • Activities require the employee to physically assist patients on an occasional basis, when stepping up onto scales, exam tables, to and from a seated position or into positions required for specific procedures.
