   <title><![CDATA[Registered Nurse (RN) Case Manager - Care Partners (Facility Based Care) (Butler/Warren County)]]></title>
   <description>&lt;p&gt;SUMMARY &lt;br /&gt;Provides nursing care and coordinates hospice care for assigned patients/families. The Care Partners Case Manager is committed to the philosophy, goals, and objectives of the hospice concept of care.&lt;/p&gt;
&lt;p&gt;ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.&lt;/p&gt;
&lt;p&gt;Makes ongoing assessment and follow-up patient visits within our partner facilities (Nursing Homes, ECF&apos;s, ALF&apos;s). Evaluates response to care provided.&lt;/p&gt;
&lt;p&gt;Develops, maintains, and implements written nursing care plans that promote 24 hour continuity of care. Also, establishes and maintains appropriate documentation of care provided.&lt;/p&gt;
&lt;p&gt;Provides case management of all patient services in planning care to meet physical, emotional, spiritual, bereavement and other needs. Consults physicians as necessary and serves as a consultant to other professionals and volunteers.&lt;/p&gt;
&lt;p&gt;Participates in bi-weekly Inter-Disciplinary Team meetings and periodic Care Coordination meetings within Partner Facilities as needed.&lt;/p&gt;
&lt;p&gt;Teaches, supervises, and evaluates care provided by volunteers, home health aides, and primary care givers.&lt;/p&gt;
&lt;p&gt;Assesses spiritual needs of patient/family members.&lt;/p&gt;
&lt;p&gt;Participates in hospice in-service and continuing education programs.&lt;/p&gt;
&lt;p&gt;Participates in Continuous Quality Improvement activities and other organizational committees.&lt;/p&gt;
&lt;p&gt;Provides on-call coverage on rotation&amp;nbsp;after hours including weekends, nights, and holidays to ensure 24 hour, 7 day a week hospice coverage to patients and families.&lt;/p&gt;
&lt;p&gt;Provides nursing care and treatments in accordance with approved nursing standards.&lt;/p&gt;
&lt;p&gt;May be rotated into other appropriate positions as business needs dictate. The responsibilities and duties of the rotational position will apply during that time.&lt;/p&gt;
&lt;p&gt;Precepts new staff.&lt;/p&gt;
&lt;p&gt;Completes educational needs to meet licensure requirements, HOD policy and JACHO requirements.&lt;/p&gt;

   <title><![CDATA[Registered Nurse - PRN (Dayton)]]></title>
   <description>&lt;p&gt;Current PRN openings are for Coming Home, our homecare team.&lt;/p&gt;&lt;p&gt;Hospice of Dayton cares for terminally-ill patients in their personal homes, patients residing in facilities such as Nursing Homes and in our In-Patient Units. We have a 51-bed In-Patient Unit in Dayton on Wilmington Avenue and a 14-bed In-Patient Unit serving Butler &amp;amp; Warren Counties in the Franklin/Middletown area. We serve patients and families in an 8 county service area including Montgomery, Clark, Greene, Miami, Preble, Clinton, Butler and Warren counties.&lt;/p&gt;&lt;p&gt;&amp;nbsp;&lt;/p&gt;&lt;p&gt;PRN Registered Nurses must be flexible, adaptable and have a strong clinical background in pain and symptom management. There is a need for an effective communicator, someone who is able to be aware of and responsive to the patient and family&apos;s needs and concerns. &lt;P&gt;SUMMARY &lt;BR&gt;&lt;BR&gt;This position is responsible for inpatient unit communications between staff and patients, and facilitation of the admission and transfer process for patients entering and leaving the Care Center and maintaining medical records. The Unit Coordinator is committed to the philosophy, goals, and objectives of the hospice concept of care &lt;BR&gt;&lt;BR&gt;ESSENTIAL DUTIES AND RESPONSIBILITIES may include the following:&lt;BR&gt;&lt;BR&gt;Incorporates the customer service value and standards into daily work activities and experiences with patients, families, visitors, physicians and co-workers. &lt;BR&gt;&lt;BR&gt;Answers telephone and intercom calls and provides information or relays messages to patients and medical staff. Directs visitors to patients&apos; rooms.&lt;BR&gt;&lt;BR&gt;Assembles charts, completes necessary forms, and sends relevant copies to appropriate departments for new admissions and inputs information into Misys&lt;BR&gt;&lt;BR&gt;Informs HIM of patient status changes and accurately inputs information into MISYS. &lt;BR&gt;&lt;BR&gt;Transcribes medical orders and re-copies medication sheets as appropriate. &lt;BR&gt;&lt;BR&gt;Completes the demise forms. Calls physicians as appropriate and informs HIS of patient&apos;s death. &lt;BR&gt;&lt;BR&gt;Expedites patient transfer process by copying and faxing necessary paperwork and communicating information by telephone. Copies and delivers documents to appropriate parties for patient discharge. &lt;BR&gt;&lt;BR&gt;Keeps accurate patient data in Misys, on the patient boards, Care Center census &amp; medical director sheets.&lt;BR&gt;&lt;BR&gt;Makes additions &amp; deletions to Care Center census, prints, and delivers copies to specific staff.&lt;BR&gt;&lt;BR&gt;Makes appropriate changes in staff assignments in Hill-Rom system and assigns locator badges.&lt;BR&gt;&lt;BR&gt;Retrieves charts from HIM open file area when patients are transferred into the HCC. &lt;BR&gt;&lt;BR&gt;After a transfer, discharge, or death prepares the chart for return to HIM.&lt;BR&gt;&lt;BR&gt;Contacts and schedules appointments for patients with dietician, physical therapy, x-rays, rehab, chemo therapy, doctor appointments, etc. &lt;BR&gt;&lt;BR&gt;Distributes mail to all staff and patients. &lt;BR&gt;&lt;BR&gt;Facilitates communication by posting memos and notices for staff information. &lt;BR&gt;&lt;BR&gt;Prepares paperwork for team meetings. &lt;BR&gt;&lt;BR&gt;Schedules transportation for patients leaving unit.&lt;BR&gt;&lt;BR&gt;Prints Care Center medication sheets on a nightly basis and as patient arrives in Care Center.&lt;BR&gt;&lt;BR&gt;Copies and files original narcotic sheets. &lt;BR&gt;&lt;BR&gt;Participates in HOD education and attends staff meetings to maintain competency. &lt;BR&gt;&lt;BR&gt;Satisfactorily performs all disaster drills. &lt;BR&gt;&lt;BR&gt;Restocks office supplies according to par levels. &lt;BR&gt;&lt;BR&gt;Follows all applicable federal and state regulations, including HIPAA, compliance and employment regulations.    <title><![CDATA[Hospital Liaison  (Dayton)]]></title>
   <description>&lt;div id="Description"&gt;The Hospital Liaison is responsible for growing key service line areas as defined by management, developing and maintaining new and current relationships with assigned hospital(s). As a critical member of the marketing and admissions teams, the Hospital Liaison works closely with hospital discharge planners, physicians, palliative team members, social workers, case managers, management, inpatient unit and education staff to generate referrals. The Hospital Liaison also provides initial and on-going assessments of patients in facility contracted beds, and manages patients admitted to the hospital for symptom management.&lt;/div&gt;
&lt;p&gt;&amp;nbsp;&lt;/p&gt;
&lt;div&gt;Four ten-hour shifts, 8:30 a.m. to 7 p.m. with occassional weekend work. Starting pay range: $47K - $60K, actual starting salary will be commensurate with experience&lt;/div&gt;

&lt;div&gt;ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.&lt;/div&gt;

&lt;div&gt;Provides an overview of hospice services and secures appropriate signed consents for admission to hospice program.&lt;/div&gt;

&lt;div&gt;Communicates with hospital staff to establish a coordinated plan of care that reflects the hospice philosophy and the patient&apos;s needs, for hospice patients who are admitted to hospitals or are awaiting admission to HOD.&lt;/div&gt;

&lt;div&gt;Coordinates the completion of the admission process and ensures that appropriate resources are obtained for a successful transfer to home care, or to an extended care facility.&lt;/div&gt;

&lt;div&gt;Provides the receiving team or facility with appropriate information to facilitate continuity of care.&lt;/div&gt;

Consults physicians as necessary and serves as a consultant to other professionals and volunteers.&lt;br /&gt;&lt;br /&gt;Teaches, supervises, and evaluates care provided by volunteers, home health aides, and primary care givers.&lt;br /&gt;&lt;br /&gt;Assesses spiritual needs of patient/family members.&lt;br /&gt;&lt;br /&gt;Facilitates continuity of care by providing a written summary of patient care when a patient is admitted to a facility or another agency.&lt;br /&gt;&lt;br /&gt;Coordinates patient care by utilizing existing agencies and community resources when appropriate.&lt;br /&gt;&lt;br /&gt;Participates in hospice in-service and continuing education programs.&lt;br /&gt;&lt;br /&gt;Participates in Continuous Quality Improvement activities and other organizational committees.&lt;br /&gt;&lt;br /&gt;Provides on-call coverage after hours including weekends, nights, and holidays to ensure 24 hour, 7 day a week hospice coverage to patients and families. &lt;br /&gt;&lt;br /&gt;Provides nursing care and treatments in accordance with approved nursing standards.&lt;br /&gt;&lt;br /&gt;May be rotated into other appropriate positions as business needs dictate.    <description>SUMMARY 

Provides spiritual counsel and emotional support to active and bereaved hospice patient/family units.  Acts as liaison between community clergy and hospice.  The Chaplain is committed to the philosophy, goals, and objectives of the hospice concept of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. 

Participates in providing bereavement services to individuals and groups.

Provides direct spiritual counsel, prayer, and ritual/sacramental ministry as appropriate and requested.

Conducts memorial/funeral services for those families not affiliated with a church or other religious organization and who desire a chaplain&apos;s participation, as schedule allows.

Assesses spiritual needs of patients/family members.

Attends IDT meetings, collaborates with IDT members and participates in developing interdisciplinary care plan for patient.

Attends Care Coordination Meetings at facilities where this is occurring

Assists members of hospice team to understand spiritual concerns related to patient/family response to dying.

Serves as liaison and resource for community clergy whose members become hospice patients.

Makes or assists family/patient in making contact with outside clergy.

Assists in developing and providing educational and public relations programs for staff and clergy community.

Participates in Continuous Quality Improvement activities and organizational committees.

Documents care provided in patient records in a timely manner.

Assists in developing, promoting, and supervising volunteer chaplaincy program.

Precepts new staff.

Participates in on-call coverage with Family Support Services.

Follows all applicable federal and state regulations, including HIPAA, compliance and employment regulations. Complies with all HOD policies and procedures and JCAHO standards.
