ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Accurately collects and documents patient data through established nursing techniques to assess patient health status. Involves the patient and/or family in collecting data. Inputs data into electronic medical record at point of care.

Identifies and prioritizes patient specific problems, appropriate interventions and realistic goals that are incorporated into the plan of care.

Provides nursing care by administering medications, intravenous therapies, blood and blood products, and treatments as ordered by the physician.

Provides specific end-of-life education, pre-term care and post-mortem care with dignity while supporting the patient/family.

Facilitates continuity of care by providing information regarding patient transfers.

Teaches, supervises and evaluates care provided by nursing assistants, LPN's and caregivers.

Ensures that documentation of care provided meets established standards. Hospice of Dayton staff are expected to complete electronic documentation at point of care.

Re-evaluates patient/family needs at regularly scheduled intervals and modifies the plan of care accordingly.

Incorporates the customer service values and standards into daily work activities and experiences with patients, families, visitors, physicians and co-workers.

Works collaboratively with other team members to coordinate care.

Satisfactorily performs all disaster drills.

Participates in team conferences and attends staff meetings.

Participates in Performance Improvement activities.

Participates in hospice education and continuing education to maintain professional competency.

Follows all applicable federal and state regulations, including HIPAA, compliance and employment regulations. Complies with all HOD policies and procedures.

SUMMARY
Provides nursing care and coordinates hospice care for assigned patients/families. The Care Partners Case Manager is committed to the philosophy, goals, and objectives of the hospice concept of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Makes ongoing assessment and follow-up patient visits within our partner facilities (Nursing Homes, ECF's, ALF's). Evaluates response to care provided.

Develops, maintains, and implements written nursing care plans that promote 24 hour continuity of care. Also, establishes and maintains appropriate documentation of care provided.

Provides case management of all patient services in planning care to meet physical, emotional, spiritual, bereavement and other needs. Consults physicians as necessary and serves as a consultant to other professionals and volunteers.

Participates in bi-weekly Inter-Disciplinary Team meetings and periodic Care Coordination meetings within Partner Facilities as needed.

Teaches, supervises, and evaluates care provided by volunteers, home health aides, and primary care givers.

Assesses spiritual needs of patient/family members.

Participates in hospice in-service and continuing education programs.

Participates in Continuous Quality Improvement activities and other organizational committees.

Provides on-call coverage on rotation after hours including weekends, nights, and holidays to ensure 24 hour, 7 day a week hospice coverage to patients and families.

Provides nursing care and treatments in accordance with approved nursing standards.

May be rotated into other appropriate positions as business needs dictate. The responsibilities and duties of the rotational position will apply during that time.

Precepts new staff.

Completes educational needs to meet licensure requirements, HOD policy and JACHO requirements.

Follows all applicable federal and state regulations, including HIPAA, compliance and employment regulations. Complies with all HOD policies and procedures and JCAHO standards.

Current PRN openings are for Coming Home, our homecare team.

Hospice of Dayton cares for terminally-ill patients in their personal homes, patients residing in facilities such as Nursing Homes and in our In-Patient Units. We have a 51-bed In-Patient Unit in Dayton on Wilmington Avenue and a 14-bed In-Patient Unit serving Butler & Warren Counties in the Franklin/Middletown area. We serve patients and families in an 8 county service area including Montgomery, Clark, Greene, Miami, Preble, Clinton, Butler and Warren counties.

PRN Registered Nurses must be flexible, adaptable and have a strong clinical background in pain and symptom management. There is a need for an effective communicator, someone who is able to be aware of and responsive to the patient and family's needs and concerns. As response-time is a critical component of our care, a faster-than-average pace will be the norm for this position.

Technical Skills used (will provide training during orientation):

Infusion therapy skills - PICC line, peripheral IV, venous access port, central catheter

CADD-Prizm ambulatory infusion pump

CoaguChek XS

Subcutaneous medication administration

Blood draws from central lines

Central line dressing changes

Compensation:

Unit Coordinator-Part Time (Dayton)

SUMMARY

This position is responsible for inpatient unit communications between staff and patients, and facilitation of the admission and transfer process for patients entering and leaving the Care Center and maintaining medical records. The Unit Coordinator is committed to the philosophy, goals, and objectives of the hospice concept of care

ESSENTIAL DUTIES AND RESPONSIBILITIES may include the following:

Incorporates the customer service value and standards into daily work activities and experiences with patients, families, visitors, physicians and co-workers.

Answers telephone and intercom calls and provides information or relays messages to patients and medical staff. Directs visitors to patients' rooms.

Assembles charts, completes necessary forms, and sends relevant copies to appropriate departments for new admissions and inputs information into Misys

Informs HIM of patient status changes and accurately inputs information into MISYS.

Transcribes medical orders and re-copies medication sheets as appropriate.

Completes the demise forms. Calls physicians as appropriate and informs HIS of patient's death.

Expedites patient transfer process by copying and faxing necessary paperwork and communicating information by telephone. Copies and delivers documents to appropriate parties for patient discharge.

Keeps accurate patient data in Misys, on the patient boards, Care Center census & medical director sheets.

Makes additions & deletions to Care Center census, prints, and delivers copies to specific staff.

Makes appropriate changes in staff assignments in Hill-Rom system and assigns locator badges.

Retrieves charts from HIM open file area when patients are transferred into the HCC.

After a transfer, discharge, or death prepares the chart for return to HIM.

Contacts and schedules appointments for patients with dietician, physical therapy, x-rays, rehab, chemo therapy, doctor appointments, etc.

Distributes mail to all staff and patients.

Facilitates communication by posting memos and notices for staff information.

Prepares paperwork for team meetings.

Schedules transportation for patients leaving unit.

Prints Care Center medication sheets on a nightly basis and as patient arrives in Care Center.

Copies and files original narcotic sheets.

Participates in HOD education and attends staff meetings to maintain competency.

Satisfactorily performs all disaster drills.

Restocks office supplies according to par levels.

Hospital Liaison (Dayton)

The Hospital Liaison is responsible for growing key service line areas as defined by management, developing and maintaining new and current relationships with assigned hospital(s). As a critical member of the marketing and admissions teams, the Hospital Liaison works closely with hospital discharge planners, physicians, palliative team members, social workers, case managers, management, inpatient unit and education staff to generate referrals. The Hospital Liaison also provides initial and on-going assessments of patients in facility contracted beds, and manages patients admitted to the hospital for symptom management.

Four ten-hour shifts, 8:30 a.m. to 7 p.m. with occassional weekend work. Starting pay range: $47K - $60K, actual starting salary will be commensurate with experience

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Assesses potential patient's condition and past medical history and obtains appropriate documentation to support hospice eligibility and ensures that the hospice certification is accurately completed.

Provides an overview of hospice services and secures appropriate signed consents for admission to hospice program.

Communicates with hospital staff to establish a coordinated plan of care that reflects the hospice philosophy and the patient's needs, for hospice patients who are admitted to hospitals or are awaiting admission to HOD.

Coordinates the completion of the admission process and ensures that appropriate resources are obtained for a successful transfer to home care, or to an extended care facility.

Provides the receiving team or facility with appropriate information to facilitate continuity of care.

Collaborates with the attending physician to effectively manage the hospitalized HOD patient and ensure that transfers to other care sites are expedited as soon as possible.

Serves as consultant to other professionals including the hospital staff by providing clinical expertise in the area of hospice care.

Consults and collaborates with physicians as appropriate.

Assists in planning and executing educational offerings for hospital staff related to the care of hospice patients.

Participates in Grand Rounds, Tumor Board, or other hospital staff related functions to communicate the hospice philosophy and HOD services.

Interfaces with facility utilization review staff to facilitate timely transfer of HOD hospitalized patients and appropriate use of facility resources.
