   <title><![CDATA[Scheduler / Patient Services Coordinator-Dayton IPU (Dayton)]]></title>
   <description>The Patient Care Coordinator works in the Resource Center and is the first line of telephone contact to appropriately answer and direct telephone contacts relative to patient care needs, and provides scheduling services to our patient&apos;s and family&apos;s as needed. A Patient Care Coordinator, once fully trained, would be expected to perform the duties in Scheduling and/or telephonic PBX operator services as needed. The Patient Care Coordinator uses appropriate scheduling techniques to best meet the needs of patients and families as well as receiving and responding to the direction given from their Director or Scheduling Coordinators. The Patient Care Coordinator is aware that duties will change, based on the changing needs of patients and families; the ultimate goal is to assist clinical services staff in meeting the customer service needs of our patients and families. The Patient Care Coordinator is committed to the philosophy, goals, and objectives of the hospice concept. &lt;BR&gt;&lt;BR&gt;ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. &lt;BR&gt;&lt;BR&gt;Shows consistent use of proper telephone etiquette according to Hospice of Dayton guidelines and expectations.&lt;BR&gt;&lt;BR&gt;Answer incoming calls, page overhead and on page gate when required to do so.&lt;BR&gt;&lt;BR&gt;Communicates effectively with customers (Physicians, families and patients, Care Partner facility staff) and with Hospice of Dayton staff including the entire Interdisciplinary Team, as necessary, by utilizing proper telephone etiquette and good customer service according to HOD expectations and protocols.&lt;BR&gt;&lt;BR&gt;Performs the primary and secondary duties defined in Scheduling Function or Triage Function as assigned&lt;BR&gt;&lt;BR&gt;Accurately enters information into the Clinical Information System to schedule visits, order equipment, or document actions or updates to the patient needs.
